Enclosed for your evaluation and review are proposals we received on the above captioned
project. Proposals were submitted by:

Additionally, please find enclosed: (1) a copy of the original RFP, (2) Written Evaluation Form,
(3) Review and Assessment criteria, (4) a copy of Resolution 2003-147 and (5)the Guidelines for
Evaluators on "How to Evaluate RFP's".
• Each member of the RFP Evaluation Committee is to rate the proposal independently.
• Complete confidentiality is an ethical and legal requirement, and is vital to fair, equitable
evaluation.
• Please remember that vendor information, proposals, results, and other evaluation
proceedings are confidential at all times.
• Upon completion of your evaluation, deliver or fax the completed Evaluation Form with the
points tallied by 4:00 PM, March 26, 2004 to the Purchasing Division (970) 221-6707.
• Interviews will be scheduled during a four (4) hour period, the week of April 1, 2004.
