(GROUND) performed during the construction of the BFCMC Temporary Modular Cath Lab at the Banner Health Facility located at 4700 Lady Moon Drive in Fort Collins, Colorado. The materials testing and special inspections were performed on a part-time basis at times scheduled by the contractor and/or subcontractor. During construction, representatives of our office were scheduled for the specific items below on the dates indicated. Structural Construction Item Elements Dates of Testing or Special Inspection Exceptions noted Foundation Bearing Soils Overexcavation under Footings and Slab Open Hole Rpt: 10/22/20 Compaction: 10/23/20, 10/29/20, 11/2/20 - Reinforcing Steel Spread Footings, Stem Walls, 11/4/20, 11/12/20 - Structural Concrete: Field/Lab properties Spread Footings/ Stem Walls 11/4/20, 11/13/20 - Structural Backfill (compaction) At Permanent Structure Footings / Stem wall 11/10/20 - Commercial Building Special Inspections Light Gauge Framing 12/8/20, 12/16/20, 12/21/20 - Fire Penetrations - Inspected by Others n/a Structural Steel – CWI (special inspections) Welded Connections 11/18/20 - Based on the results of our scheduled materials testing and special inspections referenced above and to the best of our knowledge, it is our opinion that these structural construction items were in built in general accordance with project plans and specifications, with the exceptions noted. The materials testing and special inspections as indicated above was periodically performed to provide data to the Owner, Contractor, and Subcontractors to assist them in evaluating the general compliance of the tested materials and/or work with the contract documents at the times of our observation and testing. We did not supervise, direct or have any control over the BFCMC Temporary Modular Cath Lab – Banner Health 4700 Lady Moon Drive, Fort Collins, Colorado Job No. 20-0665 Ground Engineering Consultants, Inc. We were not responsible for the Contractor’s means, methods, procedures, techniques, or sequences of construction, nor responsible for Contractor to carry out the work in accordance with the contract documents.
