SUBMITTAL REQUIREMENTS:
1) Six (6) copies of RECORDED plan (i.e. site, landscape, elevation, etc. - whichever is being altered) on
24" x 36" size (usually available at the Technical Services Department at 281 N. College) — RED -LINE
THE CHANGES ON the six plans or provide six copies of revised drawings showing the changes in
addition to the six originals;

MINOR AMENDMENT
Detailed description of the change and reason(s) for the request: verizatwireless proposes to modify ari existing con—mical ons site operated byOwest.
Exietlng antennas (2 in each of 3 sectors) will be replaced with 5 antennae in each of 3 sectors. An equipment enclosure will be added on the roof where existing equipnbM cabktets are located on a metal deck.
AN etntctrree on the root wig be the sarne corrugated metal as the eWstkq site with a frown painted finish. A generator will be added near the rear entrance of the budding. The enclosure wig be an extension of the

eds&q budding. with a facade to'match the existing building. The reason for the site modification is to improve wireless coverage in the vicinity of Foothills Mall and to update existing equipment to current standards.
