DATE: April 11, 2008
An address change has been approved for Lot 2 of Poudre Valley Hospital Second Filing.
The proposed Medical Office Building in Lot 2 will now carry an address off of S.
Lenny Ave rather than Garfield St. The address assigned for Lot 2 had been 1045
Garfield St, however, the new address of the future Medical Office Building will now be
1107 S. Lemay Ave.
The City of Fort Collins in cooperation with Poudre Fire Authority have granted this
address change because the new structure will have a pedestrian bridge connected to the
Proposed parking structure and the existing hospital campus. Both the parking structure
and the existing hospital are addressed off Lemay Ave.
Hereafter: The proposed Medical Office Building located in Lot 2 of Poudre Valley
Hospital Second Filing will no longer carry the address of 1045 Garfield St., but instead
now carries the address of 1107 S. Lemay Ave.
Please update your records to reflect this change.
