The Columbine East Long Term Care Facility was originally platted with an address of 1150 Rule
Dr. However, the address used at this location prior to the plat was 4824 South Lemay Ave. For
some reason, the address of 4824 S. Lemay Ave. had continued to be used for Lot 1, instead of the
platted address of 1150 Rule Dr. To correct for this error, the platted address for the Columbine East
Long Term Care Facility plat will now be officially changed to 4824 S. Lemay Ave.
Hereafter: Lot 1 of Columbine East Long Term Care Facility will no longer be assigned the 1150
Rule Dr. address, but instead will carry an address of 4824 S. Lemay Ave.
Please adjust your records accordingly.
