This pert'nit may be revoked by the issuing authority if at any time the permitted access approach and its use violate any of the terms and conditions of this permit or the State Highway Access Code. The use of advance warning and construction signs, flashers, barricades and flagmen are required at all times during access approach construction within State right-of-way in conformance with the MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES, Part VI. The Department of Highways, its employees and agents shall be indemnified and held harmless against any action or damageto property or persons sustained by reason of the exercise and use of this permit. SITE REQUIREMENTS 1. "Right Turn Only" signs to be installed at exit on College Avenue. 2. All Curb, gutter, and sidewalks shall be installed adjacent to property. 3. All construction shall be completed in an expeditious and safe manner and shall be finished within 45 days from initiation. The permitted access approach shall be completed in accordance with the terms and conditions of the permit (prior to being used. The permittee shall notify Dick Jensen ` with the Colorado Department of Highways in . Loveland at 667-4650 at least 48 hours prior to commencing construction within the State Highway right-of-way. The person signing as the permittee must be the owner or legal representative of the property served bythe permitted I access. Blue, Traffic En; 1. This permit is only for use and purpose stated in the Application and Permit. Any changes in 'the type and use and or volume of traffic using the access may require the revision of the permit. 2. This access serves a 2400 sq. ft. convenience store and 2400 sq. ft. business and the maximum anticipated average daily traffic is 400. 3. Driveway shall be constructed 30 feet wide with 50 foot radii. 4. The access approach shall be surfaced immediately upon completion of earthwork construction and prior to being used. 5. Surfacing shall consist of 3 inch HBP Grading Ex over 4 inch ABC Class 6, over 8 inch ABC Class 1. Specifications for these materials are attached. 6. The first 20 feet beyond the closest Highway lane, including speed change lanes, shall slope down and away from the Highway at a 2% grade to ensure proper drainage control. 7. The horizontal axis of the access shall be at a right angle to the centerline of the highway and extend a minimum of 40 feet beyond the outside edge of the nearest lane. 8. Construct handicap ramps at the intersection of sidewalks and curbs according to the attached specifications. 9. Drainage to the State Highway right of way shall not exceed the historical flow. 10. If frost is present in the sub -grade, no surfacing material shall be placed until all frost is gone or removed. 11.
