Re: Amendment No. 2 to 7601 Health and Wellness Provider- Senior Center
Dear Ms. Myers,
Please let this letter serve to amend the following to the Services Agreement dated May 13, 2014
(“Agreement”).
The following changes/additions are hereby made and incorporated in the Services Agreement:
1. The contract term will be extended for one (1) additional year, May 1, 2016 through April 30, 2017.
If the renewal is acceptable to your firm, please sign this letter in the space provided and attach a
current copy of insurance naming the City as an additional insured for General and
Automotive Liability, within the next fifteen days.
2. If this extension is not agreeable with your firm, we ask that you send us a written notice stating that
you do not wish to renew the contract and state the reason for non-renewal.
3. Contract Sum- The City shall pay the Service Provider for the performance of this Contract, subject
to additions and deletions provided herein, a revenue share percentage as per attached Exhibit “B”,
consisting of one (1) page, and incorporated herein by this reference. The City shall pay the
Service Provider on a monthly basis by the 15th business day for the City’s revenue share from the
preceding month. New programs and offerings may be added with mutual discussion and
agreement of revenue share by both parties prior to implementation.
4. Exhibit B – Compensation, attached and incorporated herein by this reference, replaces the
amended Exhibit B to incorporate updated revenue share percentages.
All other terms and conditions of the Agreement shall remain in full force and effect. The City looks
forward to continuing to work with Columbine Health.
The parties hereto have executed this Second Amendment the day and year shown.
