RE: Renewal, Miscellaneous Agreement - Cartridge Recycling
Dear Mr. Castor:
The City of Fort Collins wishes to extend the agreement term for the above captioned
proposal per the existing terms and conditions and the following:
1) The term will be extended for one (1) additional year, October 1, 2016 through
September 30, 2017.
2) Revised Exhibit A Scope of Work, as per attached.
3) Revised Bin Pickup Locations and Site Contacts, as per attached.
If the renewal is acceptable to your firm, please sign this letter in the space provided and
include a current copy of insurance certificate naming the City as an additional
insured for General and Automotive Liability within the next fifteen (15) days.
If this extension is not agreeable with your firm, we ask that you send us a written notice
stating that you do not wish to renew the contract and state the reason for non-renewal.

Changes to Exhibit A Scope of Work (revised 9/26/16):
1. Effective October 1, 2016: Cartridge bin pickups for high-usage sites will be every two
weeks, to start. Low-usage sites will be picked up monthly, to start. Pick-up frequency
will be adjusted on an as-needed basis; to be triggered by outreach by the site contact.
The service will be cost-neutral; meaning the Service Provider will provide services at no
charge to the City, and payments to City departments will also cease going forward.
2. Amanda Gomez becomes the new single point of contact for Service Provider service
issues going forward:
