QUALIFICATION STATEMENT FORM
1. Experience
Black Tie Events has been operating in Fort Collins for 5 years. In the past, we have
offered bartending services for both large and small events with exemplary execution. We
pride ourselves on professionalism, the safe service of alcohol, cleanliness, and our ability
to anticipate and exceed the expectations of our guests, clients, and venues throughout
Northern Colorado. Our owners each have over 10 years of experience in the hospitality
industry, including but not limited, to bar management and operation.
Safe Service of Alcohol:
Our sister company, the Fort Collins Academy of Bartending, has been an active member
of, and has worked closely with Nathan Dewey at Team Wellness & Prevention’s RAR
program. As part of this program, we require all staff members’ successful completion of
both TIPs On-Premises Certification, as well as the Fort Collins Academy of Bartending’s
Certificate of Completion of Bartending 101, which focuses on responsible service of
alcohol, and tests it’s graduates on accurate and efficient free pouring.
Cleanliness:
Throughout the last decade, we have had the privilege of assisting bar and restaurant
owners throughout our region, design their bars for maximum efficiency, train & recruit
staff, and manage day-to-day operations. We have worked successfully with the Larimer
County Health Department in an effort, to not only pass initial health inspections, but to
assist staff members and owners continue down that path of success. We understand the
vital responsibility we as vendors have in keeping our communities safe, and healthy.

Our experience, paired with the valuable relationships we have developed with beer &
liquor purveyors, non-profit organizers, and community members, have attributed to years
of success for our team. We have both the relationships and systems in place to execute
exemplary service to the clients and guests at each event center. We invite you to reach
out to both our former and on-going clients, for a glimpse of the exceptional level of
service our team continuously provides.
2. Financial
We have a record of success for all of our businesses; Black Tie Events, Fort Collins
Academy of Bartending, and Rocky Mountain Sno Caps. We are also more than willing to
personally fund any licensing, administrative fee, and operational costs associated with
the success of fulfilling this contract.

ATTACHMENT B
CONCESSION FEE PROPOSAL FORM
Option I:
The Concessionaire shall pay, annually, as rental to the City for concession operations at the
Senior Center ten percent (10%) of the gross sales from the concession operation.
The Concessionaire shall pay, annually, as rental to the City for concession operations at Club
Tico ten percent (10%) of the gross sales from the concession operation.
The Concessionaire shall pay, annually, as rental to the Northside Aztlan Community Center
(NACC), ten percent (10%) of the gross sales from the concession operation.
Gross sales are defined as total concession sales excluding sales tax.
I hereby acknowledge that I will comply with all terms and conditions as stated, explained in
RFP 8683 Event Beverage Services – Recreation Centers, and to the Terms and Conditions
of the attached sample Agreement, including all Insurance and Liquor License Requirements.
