Structural excavation  Cast in-place box installation  Inlet and stormwater pipe installation 9 SUMMARY OF WORK  Waterline lowering: 4” to 8” back to 4” with casing  Concrete curb and gutter, access ramps  Asphalt milling / overlay and full depth asphalt pavement  Parkway irrigation and turf restoration  Traffic control 10 SUMMARY OF WORK  Method for Handling Traffic  All Plans must be approved by Syl Mireles – COFC Traffic 11 SUMMARY OF WORK  Construction Staking  Provided by the City of Fort Collins  48 Hours (Minimum) Required for Notification  Testing  Paid for by the City of Fort Collins thru CTL Thompson  Contractor will coordinate w/ City PM, testing as required  Striping  Provided by the City of Fort Collins  Floodplain Use Permit  Will be obtained and managed by Engineering 12 PROJECT COORDINATION  Area residents  Access to temporary parking  Maintaining safe pedestrian access at all times  Touchstone Facility  South East corner  Fairly constant stream of patients / customers  City Traffic / Light & Power  Grade adjustment of manhole access on NW corner.  Xcel Energy  No known conflicts at this time 13 HIGHLIGHTS OF GENERAL CONDITIONS  Superintendents The General Contractor SHALL employ a superintendent whose sole responsibility is to monitor and coordinate all facets of the work, including project decision making. 14 HIGHLIGHTS OF GENERAL CONDITIONS  Substantial Completion - Defined • Box Removals, Utilities, Structural Excavation, Box installation, Backfill -- April 15th, 2015  Liquidated Damages – Substantial Completion If the Contractor goes past the 100 calendar days* for substantial completion, damages in the amount of One Thousand Four Hundred Dollars ($1400.00) for each calendar day will be assessed 15 HIGHLIGHTS OF GENERAL CONDITIONS  Final Acceptance - Defined • Completion and Acceptance of: Completion of Sidewalks, Curb and Gutter, Asphalt Patching / Paving, Landscape Restoration, Removal of Tree Protection, Removal of Erosion Control Items (except those as directed by the City to remain in place), Irrigation / Landscape and Punch List Items  Liquidated Damages – Final Acceptance If the Contractor goes past the 20 calendar days for final acceptance, damages in the amount of One Thousand Four Hundred ($1400.00) for each calendar day will be assessed 16 HIGHLIGHTS OF GENERAL CONDITIONS  Project Coordination Meetings  Weekly On-Site Meetings (including ticket collection)  Contractor to update Schedule weekly. The Contractor may be required to submit additional Schedules dependent upon progress of the project. 17 HIGHLIGHTS OF GENERAL CONDITIONS  Construction Hours  Standard Working Hours are from 7:00am to 6:00pm Monday through Friday  Weekend Work and / or Extended Hours Work may be granted. Requests for either must be made in Writing a minimum of 72 hours in advance. Requests are subject to approval by the City of Fort Collins. 18 HIGHLIGHTS OF GENERAL CONDITIONS  Haul Routes  Request to keep construction related traffic off of Whitcomb as much as possible.  Site Clean Up  The construction work shall be kept and accomplished in a neat, clean and orderly fashion  Street sweeping may be required in order to minimize vehicle tracking and to meet EPA storm water quality requirements  Dust control will be required as necessary 19 TRAFFIC CONTROL  Contractor Responsibilities:  Maintain vehicle, bicycle, and pedestrian traffic at all times. ALL detours for ALL modes of transportation shall be approved by the City prior to construction.  Maintain access to all local businesses  Review all upcoming lane closure needs and /or changes in traffic control set up at Weekly Progress Meetings  Traffic Control Plans need to be submitted to the City seven (7) days prior to setup 20 EROSION CONTROL - SWMP  Contractor Responsibilities:  Erosion Control in accordance with Sheets 24 - 27  The Contractor is responsible for maintaining and repairing all erosion control devices throughout the project  Contractor will designate a SWMP Administrator prior to the start of construction. The SWMP Administrator will conduct weekly inspections and submit a signed, log of activity weekly to the City.
