As indicated in on page 3 of the bid document (copy attached), vendors must complete and submit certification for Buy America and Lobbying with their bid. These forms were included in Attachment 1 of the bid document. Vendors responding to this bid must comply with the federal requirements in Attachment #1. Vendors must complete and submit certification for Buy America and Lobbying with their bid. Quantities The City of Fort Collins will order a total of 7 poles for this bid. The Colorado Department of Transportation (CDOT) S-614-40A specification shall be used for 5 poles and mast arms with luminaires. The CDOT S-614-40 specification shall be used for 2 double mast arm poles with luminaires. The finish all of the poles shall be galvanized. See bid schedule for mast arm details. All bidders must provide drawings and written documentation that these drawings are approved by CDOT for bids to be considered. To be qualified, manufacturers must be pre -approved by the Colorado Department of Transportation Bridge Division. All poles shall include base sections, anchor bolts with steel template, and all hardware necessary to make a complete assembly.
