RE: Contract Renewal, DAS Maintenance for Police
Dear Mr. Dyke:
The City of Fort Collins wishes to extend the agreement term for the above captioned
proposal per the existing terms and conditions and the following:
1) The term will be extended for one (1) additional year, November 1, 2017
through October 31, 2018.
2) Exhibit A Scope of Work and Compensation, attached, per Services
Agreement dated November 7, 2016.
If the renewal is acceptable to your firm, please sign this letter in the space provided
and include a current copy of insurance certificate naming the City as an
additional insured for General and Automotive Liability within the next fifteen (15)
days.
If this extension is not agreeable with your firm, we ask that you send us a written notice
stating that you do not wish to renew the contract and state the reason for non-renewal.

EXHIBIT A
SCOPE OF WORK & COMPENSATION
Serviced System
Mobile Access (OEM) -based wireless Distributed Antenna System (DAS), related fiber-optic cabling and
distribution; related coaxial cabling, antennas, and other passive RF components; and related active
electronics, in-whole providing in-building Verizon commercial wireless coverage and 800MHz Public
Safety coverage
Special Provisions
Preventative Maintenance Work to be performed during Client’s Hours of Operation. Break-Fix
Maintenance Work that may impact System’s performance will be mutually scheduled between Client and
Centerline.
Services include:
Centerline labor to do Work;
Centerline tools required to do Work; and
Centerline travel expenses required to do Work; any Site visits beyond Service Limitations,
above, will be mutually agreed to and invoiced to Client as they occur.
Services exclude:
The cost of any firmware or software required to maintain the System
The cost of replacement hardware equipment not covered by OEM’s warranty.
The shipping costs of replacement equipment.

If additional services required –
Labor rate – Field Technician - $ 90.00/ hour
Material markup – 10%
