WORK ORDER NUMBER: PROJECT TITLE: 17 HH Do It Yourself Enhancements ORIGINAL BID/RFP NUMBER & NAME: 7572 FORTZED Marketing & Community Engagement MASTER AGREEMENT EFFECTIVE DATE: =December 19, 2013 WORK ORDER COMMENCEMENT DATE: 12/01/2017 WORK ORDER COMPLETION DATE: 12/31/2018 MAXIMUM FEE: (time and reimbursable direct costs): $8,000 PROJECT DESCRIPTION/SCOPE OF SERVICES: A-Train Marketing will work with City staff to identift., and implement the highest value enhancements to the Healthy Homes DIY Assessment Website. Services include: Strategic Planning: Participate in process mapping brainstorming session with FC-LEAN to Identify and prioritize improvements - Work with Healthy Homes team to finalize improvements and costs, and create implementation plan Enhancement Design and Programming: Enhancements selected after the Strategic Planning phase are likely to include one or more of the following: - Automatically generate results reports to send to DIV participants - Add enhanced ability for users to contact the City team during their assessment, and to provide feedback on the DIV assessment tool and process Enhance user profile and functionality: ability for users to log and track improvements, change profile information and conduct and connect/control multiple assessments Enhance reporting capabilities for City staff: auto-generated reports, attach multiple records to a single user/email, track user resets and other actions - Set up additional email auto-sends: reminders to complete the assessment, send out a post-assessment survey link, and others; add ability to detect if emails are opened and links clicked Bi-lingual functionality (Spanish) Cttyof �s Work Onter Form OfBcl,I Putehaflng Fonn L.ast updated 1tv2017 Service Provider agrees to perform the services identified above and on the attached forms in accordance with the terms and conditions contained herein and in the Master Agreement between the parties.
