Reason for change: additional work identified in association with the construction and operation of the CCB project 2. Description of Change: additional site work identified during valve installation for the CCB project. When the new piping and valves were installed, the existing valves did not operate and function correctly, they did not completely “seat” and operated difficultly. With the CCB installed and operating, these dysfunctional existing valves impact the operation and maintenance of the CCB. Two new valves, a new meter on the clearwell bypass, manholes for actuator access, and the UTV for cleaning inside the CCB will be purchased and installed. 3. Change in Contract Price: with three change orders, including this one at $362,636, the new total for business unit 5020331000 amounts to $7,686,255.00. The total contract cost for PO 9162138 is $8,055,006.00 4. Change in Contract Time: With these new work items, the master schedule for the substantial completion is the CCB construct will change to March 2018. Abandon YDV 911 opened and in place. 36 inch valve (replaces valve CE) Provide personnel and equipment to assist in design, investigative potholing, excavation, removal of existing pipe and flange to MJ adaptor, install new valve (requiring a welded steel flange and a Pratt valve w/additional minor pipe appurtenances and no additional fittings) backfill of pipe and valve and seeding as required. 36 inch flow meter installation Provide personnel and equipment to assist in design, investigative potholing, excavation, removal precast vault lid, removal of existing meter, reassembly of pipe with new meter, installation of new precast vault lid and seeding as required. Sitework Provide personnel and equipment to load and haul City provided topsoil from stockpile site to the Water Treatment plant and spread as necessary. Install 6 precast manholes and ladders on existing valve actuators and purchase electric utility vehicle for access in the CCB.
