PROJECT DESCRIPTION/SCOPE OF SERVICES: Preconstruction suooort services for
Horsetooth and College project. See attached scope of services for more detail.
Service Provider agrees to perform the services identified above and on the attached forms in
accordance with the terms and conditions contained herein and in the Master Agreement between
the parties. In the event of a conflict between or ambiguity in the terms of the Master Agreement
and this Work Order (including the attached forms) the Master Agreement shall control.
The attached forms consisting of three Q) page(s) are hereby accepted and incorporated herein,
by this reference, and Notice to Proceed is hereby given after all parties have signed this
document.

Re: Further explanation of roles/assignments for Preconstruction Services
Dear Dan,
Please see the following summary of the roles and explanation of their specific assignments in relation to the
CMGC preconstruction services. After your review please feel free to contact me to discuss any modifications,
clarifications, or concerns.
1. Kurtis Frailey (Construction Manager), rate of $155/hr @ estimated 30hrs = $4,650.00
a. The following tasks will be performed or assisted on by Kurtis Frailey:
i. Review of phasing drawings (specific to the College/Horsetooth Intersection in relation
to concrete paving operations).
ii. Provide technical data and constructability reviews of Value Engineering for the use of
CTS concrete mixes and use of volume metric concrete mixers for high early concrete
2. Tony Bruski (Project Manager), rate of $135/hr @ 150hrs = $20,250.00
a. The following tasks will be performed or assisted on by Tony Bruski:
i. Participate in all project meetings with the City and designers
ii. Constructability Reviews
iii. Provide Value Engineering with regards to bridge and roadway construction
iv. Verification of subcontractor and material supplier scopes and ability to perform, focus
on local vendors and subcontractors
v. Review of phasing drawings
vi. Assessment of Risk and Risk mitigation
vii. Overall coordination of the MSC project team and task assignments
3. Dave Keefe (Project Superintendent), rate of $125/hr @ 50hrs = $6,250.00
a. The following tasks will be performed or assisted on by Dave Keefe
i. Participate in necessary meetings with the City and designers
ii. Constructability Reviews
iii. Focus on existing utility conflicts
iv. Focus on MHT and MHP (pedestrian) detours and phasing
4. Eric Bodyfelt (Esitmator/Scheduler), rate of $115/hr @ 55hrs = $6,325.00
a. The following tasks will be performed or assisted on by Eric Bodyfelt
i. Participate in necessary meetings focused on pricing and schedule
ii. Develop project schedule and schedule analysis for major plan changes
iii. Develop project cost estimate and analysis of cost impacts for major plan changes,
review with Independent Cost Estimator
iv. Solicit subcontractors and vendors

v. Ensure DBE and local participation goals are meet
vi. Gate Keeper of the Risk Registry
5. Nick Kakasenko (Safety Manager), rate $105/hr @ 15hrs = $1,575.00
a. The following tasks will be performed or assisted on by Nick Kakasenko
i. Constructability review discussion with Dave Keefe highlighting any potential safety
concerns that should be addressed in design or specifications
1. Traffic Control
2. Confined Space
3. Shoring (if needed)
4. Pedestrian detours
5. Night work/lighting
6. Etc.
