DESCRIPTION: APP Design Contract
1. Reason for change:
The Scope of Work has changed on this project and the artist needs additional time in the design
contract. The art project was expanded to include a Phase II.
2. Description of Change:
The artist will work with the Project Team on a Phase II of the art project. The artist will need
additional design time to develop the next phase of the art project which has an educational
Utility message.
3. Change in Contract Cost:
$1,500 will be added to the contract to give a total of $2,500 for the design contract for the Twin
Silo Park Project. Funds will be from APP Stormwater 504.5040100000.569010.6
4. Change in Contract Time:
The new contract deadline is September 29, 2017 (If this Change Order is not signed and
completed, the deadline remains September 16, 2016)
