study.
Scope of Services: Services shall include environmental hazardous material assessment including asbestos survey,
lead based paint assessment, sampling, analyses, and reporting.
Contractor agrees to perform the services
identified above and on the attached form
in accordance with the terms and
conditions contained herein and in the
Construction Work Order Agreement
between the parties. In the event of a
conflict between or ambiguity in the terms
of the Construction Work Order
Agreement and this work order (including
the attached forms) the Construction Work
Order Agreement shall control.
Notice to Proceed is hereby given upon
receipt of a fully executed Work Order.
