(LTE) is pleased to submit this proposal to the City of Fort Collins (the City) to provide a hazardous-material assessment for two existing structures (currently in use) to for purpose of renovation of the following:  Water Quality Lab at the Water Treatment Facility, 4316 West Laporte Avenue, and  Pollution Control Lab at the Drake Water Reclamation Facility, 3036 Environmental Drive. An alternatives analysis is being completed for both buildings to determine the future use of the buildings as labs and to understand the cost related to possible asbestos, lead and hazardous material removal associated with the renovation of the building, LTE has developed the following scope of services. SCOPE OF SERVICES LTE has separated the proposed scope of services into four tasks. These tasks include the following: Task 1 – Comprehensive hazardous-material assessment; Task 2 – Comprehensive asbestos-containing material (ACM) survey; Task 3 – Lead-based paint assessment; and Task 4 – Reporting and Cost Estimate. Page 2 Task 1 – Hazardous Material Assessment LTE will conduct initial walk-through inspections of the two buildings (interior and exterior) to assess and inventory the presence of universal hazardous-waste materials regulated by the Colorado Department of Public Health & Environment (CDPHE) Universal Waste Rule as defined in the Colorado Hazardous Waste Regulations 6 CCR 1007-3 Part 273. The materials defined in this rule include:  Batteries;  Pesticides;  Mercury-containing devices;  Mercury-containing light wastes;  Aerosol cans; and  Electronic devices and components. The CDPHE Universal Waste Rule can be review at the following web-link: https://www.colorado.gov/pacific/sites/default/files/Part-273-eff-03-30-10.pdf LTE will also assess and inventory lighting ballasts in conjunction with the CDPHE Compliance Bulletin Hazardous Waste Lighting Waste for the following:  Polychlorinated biphenyl-containing lighting ballasts in accordance with the Toxic Substance Control Act; and  Di (2-ethylexyl) phathatlate-containing lighting ballasts in accordance with the Comprehensive Environmental Response, Compensation, and Liability Act. The CDPHE Compliance Bulletin Hazardous Waste Lighting Waste can be reviewed at the following web-link: https://www.doi.gov/sites/doi.gov/files/migrated/greening/procurement/upload/CO-lighting.pdf LTE will also assess and inventory any components that are suspect of containing ozone- depleting compounds as defined in the CDPHE Air Quality Control Commission Regulation Number 15 Control of Emissions of Ozone-Depleting Compounds 5 CCR 1001-19 for the following:  Stationary appliances including any refrigeration and air-conditioning equipment that contains and uses an ozone-depleting compound refrigerant, which is not portable by nature or design. Page 3 The CDPHE Air Quality Control Commission Regulation Number 15 Control of Emissions of Ozone-Depleting Compounds 5 CCR 1001-19 can be reviewed at the following web-link: https://www.colorado.gov/pacific/sites/default/files/5-CCR-1001-19.pdf Finally, LTE will assess and inventory any items within the two buildings that may contain compounds which fall under the CDPHE Colorado Hazardous Waste Regulations Part 261– Identification and Listing of Hazardous Waste, Subpart D – Lists of Hazardous Wastes. The chemical compounds considered hazardous waste are from non-specific sources and are commonly associated with cleaning materials, chemicals, and waste streams associated with operation-and-maintenance activities conducted at the two buildings. The CDPHE Colorado Hazardous Waste Regulations Part 261– Identification and Listing of Hazardous Waste can be reviewed at the following web-link: https://www.colorado.gov/pacific/sites/default/files/Part%20261%20eff%2006-30-15.pdf LTE will provide a qualified environmental professional to perform the walk-through inspections. LTE anticipates that the initial inspections can begin within two business days after receiving notice to proceed from the City and can be completed at the same time as the ACM survey (Task 2). Task 2 – Asbestos Containing Material Survey LTE will conduct a walk-through inspection of the two buildings (interior and exterior) to assess the presence of ACM. LTE will provide a Certified Asbestos Building Inspector (CABI) to conduct the surveys. The CABI will conduct non-destructive sampling of materials potentially containing friable ACM, Category I non-friable ACM and Category II non-friable ACM. The asbestos surveys will be completed in accordance with Colorado Air Quality Control Commission Regulation 8, The Control of Hazardous Air Pollutants, Part B, Asbestos Control, and Sections III.A.1 and IV.C.1, IV.D, and IV F. The surveys will include:  An inventory of the locations of homogeneous areas where samples are collected, exact locations where each bulk sample is collected, dates that samples are collected, and homogenous areas where friable suspected ACM and homogeneous areas where non- friable ACM are present.  Samples will be collect in a statistically-random manner that is representative of the homogenous areas. Bulk samples will be collected from each homogenous area as required for suspect surfacing materials, thermal system insulation, and miscellaneous materials.  The CABI will assess if suspect ACM is friable or non-friable and classify the suspect ACM as being in good condition, damaged, or significantly damaged, and the potential for further damage. The bulk samples will be submitted to Reservoirs Environmental, Inc. (Reservoirs) in Denver, Colorado, for analysis by polarized light microscopy. LTE expects that up to 20 of the bulk samples will require further analysis by point-count methods. Reservoirs will conduct ACM analysis under standard turnaround times. LTE will notify the City if additional sampling and analysis is required. LTE anticipates that the ACM inspection can be completed in two and half days. Task 3 – Lead-based Paint Survey LTE understands that the City intends to potentially renovate the two buildings; therefore, LTE is proposing to complete a lead-based paint survey. LTE will utilize an X-ray fluorescence (XRF) to analyze lead content in painted surfaces throughout the two buildings. The XRF is a non- destructive tool limiting damage to various surfaces. LTE will provide a qualified environmental professional to perform the XRF testing. LTE anticipates the lead-based paint survey with the XRF can be completed within one day. Task 4 – Reporting LTE will provide the City with a summary letter report upon completion of the Site walk-through, sampling and analysis. The report will include details of the Site walk-through inspections, summary tables listing hazardous materials, ACM and lead sample descriptions, laboratory analytical reports, and recommendations for additional actions. LTE will also provide removal costs for identified asbestos, lead-based paint and hazardous building materials from LTE will provide the summary report within approximately 10 business days upon receiving final laboratory analysis reports. COST ESTIMATE AND ASSUMPTIONS The cost to complete the three tasks listed above is estimated on a time-and-materials basis, not to exceed $7,441.50. The cost provided is based on the following assumptions:  Scheduling can be arranged so that the assessment of the Site can be completed within three and half business days;  The Subject Property is fully accessible; DocuSign Envelope ID: 0E351DB5-3288-49E9-A287-3B4972064839 Paquette, S. Page 5  The proposed time and sample quantities detailed in this proposal are estimated based on LTE’s previous experience conducting hazardous material, ACM, and lead surveys. These quantities are for costing purposes only. LTE will notify the City if the proposed schedule and/or sample quantities detailed in this proposal are exceeded; and  The asbestos survey will not include destructive investigation behind wall, boilers, and other mechanical equipment. CLOSING LTE will perform the tasks set forth above in a thorough and professional manner. LTE cannot guarantee and does not warrant that this assessment will reveal all adverse health, safety, or environmental conditions affecting the Site. Nor can LTE warrant that the assessment requested will satisfy the dictates of or provide a legal defense in connection with any health, safety, or environmental law or regulation.
