RE: Renewal, 8076 Backflow Assembly Testing & Repair
Dear Mr. Baptista:
The City of Fort Collins wishes to extend the agreement term for the above captioned proposal
per the existing terms and conditions and the following:
1) The term will be extended for one (1) additional year, April 1, 2017 through
March 31, 2018.
2) Revised contract rates, effective April 1, 2017, as follows:
Provide testing of any size PVB – $25.85 per test
Provide testing of any size RP – $31.67 per test
Repair labor rates – $25.75 per hour
Maximum material mark-up remains the same at:
<$500 is 10%
>$500 is 8%
If the renewal is acceptable to your firm, please sign this letter in the space provided within the
next fifteen (15) days.
If this extension is not agreeable with your firm, we ask that you send us a written notice stating
that you do not wish to renew the contract and state the reason for non-renewal.
