services identified above and on the attached
forms in accordance with the terms and
conditions contained herein and in the
Services Agreement between the parties. In
the event of a conflict between or ambiguity in
the terms of the Services Agreement and this
Work Order (including the attached forms) the
Services Agreement shall control.
The attached forms consisting of one (1)
pages are hereby accepted and incorporated
herein, by this reference, and Notice to
Proceed is hereby given.

SCOPE OF SERVICES
Service Provider will conduct a workplace investigation at the direction of the Fort Collins
Human Resources Department on behalf of the City of Fort Collins.
The Service Provider will perform an objective and neutral investigation into a complaint
by an employee concerning terms and conditions of employment.
The investigator will develop a comprehensive record of allegations and evidence of
supporting and refuting information to enable the City Attorney’s Office to provide legal
counsel.
All documents will be marked “Privileged” and “Confidential.” Employees will be asked to
assist the investigator in drafting individual summary statements to represent individual
perspectives in a factual manner.
All participants will be informed of the investigator’s purpose in this process, the City’s’
expectation to keep the matter confidential where deemed appropriate, the City’s
prohibitions against retaliation for initiating this process or participating in the
investigation and the City’s expectation that he or she be entirely truthful.
The investigation shall be completed by January 31, 2017 and returned to the City’s
Chief Human Resources Officer.
