Re: Amendment No. 1 to Emergency Responder Trauma Counselors, LLC Services Agreement
Dear Ms. Rupert,
Please let this letter serve to amend the following to the Services Agreement dated September
17, 2014 (the “Agreement”).
The following changes/additions are hereby made and incorporated in the Services Agreement:
Section 5 Contract Sum – The City and/or PFA shall pay the Service Provider for the
performance of this Contract, subject to additions and deletions provided herein, Two
Thousand Dollars ($2,000.00) per month.
All other terms and conditions of the Agreement remain in full force and effect. The City looks
forward to continuing to work with Emergency Responder Trauma Counselors, LLC.
