DESCRIPTION: APP Project
1. Reason for change: Artist needs contract deadline to be extended and to include additional
work on a stone bench at the Utility Administration Building.
2. Description of Change: This extends the time line of the project. It will also expand the
Scope of Work to include carving and sandblasting a stone bench that is located in front of
one if the relief walls on site (see attached image).
3. Change in Contract Cost: An additional $5,000 will be added to the contract to complete the
carving and sandblasting of the stone bench.
4. Change in Contract Time: The contract deadline is September 9, 2016 (If this Change Order
is not signed and completed, the deadline will remain August 31, 2016)
