Proposal To: Chad Mapp From: Nate Challenger Company: City of Fort Collins Date: 8/30/16 Email: cmapp@fcgov.com Pages: 4 Phone: (970) 221-6227 Re: Utility Services Building – Phase 3 & 4 Asbestos Abatement Based on a site visit, Risk Removal has prepared the following proposal to perform selective demolition within the Utility Services Center Building located at 700 Wood Street in Fort Collins. Various asbestos containing materials (ACM) and OSHA regulated materials will be removed from the building as part of a large renovation project. Project Scope Risk Removal assumes that the remaining asbestos abatement activities will be separate into two phases that will include the following: “Phase 3” (areas that are mostly north of C400B & upstairs level) - Perform facility component removal of “brown” asbestos sealant by removing portions of the metal ductwork along with the ACM (C500A and R503) - Perform facility component removal of “red” asbestos sealant by removing portions of the metal ductwork along with the ACM (C500A and R503) - Provide up to 10 CMU block penetrations for areas that are marked by the G.C. (approx.. 4-6 square feet of block will be removed per penetration area) - Remove floor tile and mastic down to the concrete sub floor (Room 362) - Remove rolled carpet flooring, non-asbestos carpet adhesive, and remnant asbestos mastic down to the concrete sub floor (approximately 12,060 square feet from multiple locations, includes upstairs level) “Phase 4” (areas that are mostly south of C400B) - Perform facility component removal of “brown” asbestos sealant by removing portions of the metal ductwork along with the ACM (throughout multiple locations) - Perform facility component removal of “red” asbestos sealant by removing portions of the metal ductwork along with the ACM (throughout multiple locations) - Remove rolled carpet flooring, non-asbestos carpet adhesive, and remnant asbestos mastic down to the concrete sub floor (approximately 13,831 square feet from multiple locations) The asbestos duct sealant will be removed by cutting a portion of the metal ductwork in order to remove the horizontal seems and vertical seems along with adhered asbestos duct sealant. For smaller patches of 6250 Iron Forge Road  Timnath, CO 80547 (970) 221-9121  Fax: (970) 493-7446 asbestos sealant, Risk Removal will scrape the remnant material from the surface of the metal ductwork. As coordinated with the G.C., the clean metal pieces may be left within the building to be collected and discarded by the G.C. The asbestos adhesive and surface flooring will be removed up to the edges of CMU block walls, non- asbestos transition flooring (beige patterned 12”x12” floor tile), and temporary walls/doorways that were installed by the General Contractor. The vinyl cove base will also be removed from these areas in order to fully access the flooring materials. The remnant asbestos mastic will be removed down to the cement substrate for replacement flooring. Prior to Risk Removal’s mobilization for each project Phase, the G.C. and the owner will be responsible for removing all furniture, contents, and fixtures that block access to these asbestos materials. Also, the G.C. will plan to remove acoustic ceiling tile and the metal grid system for rooms where duct sealant and asbestos flooring were identified by Walsh Environmental. For identified rooms and corridors where asbestos flooring is the only hazardous material present, Risk Removal assumes that the acoustic ceiling tile and grid system will remain in place to be covered and sealed during these abatement activities. For each containment work area, Risk Removal assumes that adequate power and water supply connections will be coordinated with the G.C. Proper vent locations will be arranged with the owner and G.C. to vent the negative air machines to the exterior of the building. Risk Removal’s abatement plan includes installing 5 to 6 containments for removing the duct sealant and floor mastic for “Phase 3” and 5 to 6 containments for removing these materials with “Phase 4”. Risk Removal will perform the asbestos duct sealant and mastic abatement in modified secondary containments that include critical barriers, negative air pressure, and sealed splash guards to protect the walls. Additional containments will be installed by Risk Removal to perform the CMU block penetrations in these areas of the building. All waste material will be properly containerized, manifested and transported by Risk Removal personnel to an EPA-approved landfill for disposal. Alternate #1: OSHA Drywall: During the mobilization for each of the containments for Phase 3 and Phase 4, Risk Removal will remove small sections of OSHA drywall (approximately 6-10 square feet each) for HVAC and mechanical equipment to pass through the drywall ceiling. The drywall does not contain asbestos; however, the tape and/or joint compound contains greater than 1% (percent) chrysotile asbestos and will be removed from within the established secondary containments used to remove the flooring materials. Risk Removal assumes that the G.C. will have the drywall penetrations marked and clearly identified prior to the scheduled abatement work. Also, the G.C. will be responsible for removing the acoustic ceiling tile and metal grid to allow access to these drywall areas. Risk Removal assumes that up to 100 square feet of OSHA drywall may need to be removed from within each containment area. Project Cost The cost to complete the base scope includes the following: Phase 3 Abatement Work- $60,900.00 Phase 4 Abatement Work- $71,850.00 Each price includes planning and project design, labor, material, equipment, tools, $5,000,000.00 general liability/pollution/umbrella insurance, applicable permitting with the Colorado Department of Public Health and Environment, OSHA air monitoring throughout the removal, and waste disposal at an EPA-approved site. Risk Removal has not included final asbestos air clearance testing with the price above. Risk Removal assumes that LT Environmental will be hired by the City of Fort Collins to perform applicable clearance testing throughout the abatement work area. Line Item Pricing for Alternate #1: The price to remove up to 100 square feet of OSHA drywall from a secondary containment work area (installed to remove asbestos flooring) is $700.00 per containment work area. Project Timing The amount of ACM to be removed requires a permit with Colorado Department of Public Health and Environment. Each permit requires a 10-day period from the time of permit application to the start of work on the project. Risk Removal is prepared to promptly submit the permit applications and can mobilize upon receipt of a notice to proceed. Clarifications The owner will provide sufficient power and water in order to perform the abatement work. Access to the work areas listed above will be blocked off during the abatement work. Replacement materials are not within Risk Removal’s project scope. For projects that exceed 3,000 square feet of impacted regulated asbestos material, CDPHE regulations require the building owner to hire an independent certified asbestos project manager. This requirement may be waived if the owner is willing to sign a waiver form, and if the General Abatement Contractor meets the waiver qualifications. Risk Removal meets these qualifications. Prior to Risk Removal’s mobilization, the work areas will be cleared of office furniture, equipment, fixtures and other contents or obstacles blocking access to the identified asbestos materials. Risk Removal’s base scope of work is limited to the removal of the asbestos containing materials as described above. Removal of other types of asbestos or other hazardous materials from other areas not described may require a revision to this proposal. Risk Removal will need to attach poly sheeting on walls in order to build an effective containment. While every effort will be made to minimize paint damage when taking down the containment, it is not unusual for small patches of paint damage to occur. Risk Removal has not made provision for spot, touch-up painting, if necessary, in its Base Scope proposal. All of Risk Removal’s abatement is completed by experienced, credentialed staff, using removal methods that are in full compliance with EPA, OSHA, and Colorado Regulation 8. All waste material will be properly containerized, manifested and disposed of at an EPA-approved site Risk Removal employs only experienced, certified employees. Risk Removal completes all abatement work using state-of-the art practices and in full compliance with EPA, OSHA and State requirements. Thank you for considering Risk Removal for this project.
