HomeMy WebLinkAbout538689 HUMSTONE CONSULTING - CONTRACT - RFP - 8219 LOOMIS ADDITION SURVEYAmendment #01 8219 Loomis Addition Survey Between The City of Fort Collins and Humstone Consulting This Amendment (“Amendment”) is entered into by and between Humstone Consulting (the “Professional”) and the City of Fort Collins, Colorado (the “City”). WHEREAS, Professional and the City have entered into a Professional Services Agreement dated February 17, 2016 (the “Agreement”); and WHEREAS, the parties wish to clarify certain provisions regarding the Scope of Services and Contract Period under the Agreement; NOW, THEREFORE, in consideration of the foregoing recitals and the mutual promises herein contained, the parties agree as follows: Changes to Professional Services Agreement: • Change the Contract Period end date from ”December 31, 2017” to “December 3, 2017” . Also add a statement that, “No work under this Agreement will or has been done outside of these dates.” • Also add a statement that says: “This Agreement is for work approved by the State Historical Fund (SHF) and included in the Scope of Work for SHF contract 2016-M1-031, Loomis Addition Survey. • Add the following statement, “All work must meet the Secretary of the Interior’s Standard’s for the Treatment of Historic Properties”. Changes to Exhibit A Scope of Services: Reconnaissance survey forms (Form 1417) By July 31, Professional will submit all reconnaissance survey forms completed as of July 15, 2016. These include forms for all of Laporte Avenue and Mountain Avenue, the 100 block of North Whitcomb, North Loomis, North Grant and North Washington Streets; the 100 and 200 blocks of South Grant and 309 and 316 South Grant, totaling 107 forms. These will be reviewed by the City and returned to the Professional for final editing if necessary. These 107 forms will be reviewed per requirements of the standard History Colorado Form 1417 as approved by the City and History Colorado at the start of the project. All subsequent reconnaissance-level survey forms will be completed using a revised 1417 form (attached) as specifically requested by the City. Professional will complete this form for all remaining properties in the Loomis Addition, with the exception of: 1) 32 properties for which Professional prepares an intensive-level survey form (Form 1403); 2) 14 properties which are already designated as Fort Collins Landmarks, and therefore already have detailed information on file; and, were not surveyed prior to July 15, 2016; DocuSign Envelope ID: 1C60C3E6-5B79-4ECF-9569-EBED9956A5B2 3) 12 properties with primary buildings constructed after 1975 that were not surveyed prior to July 15, 2016; 4) Properties on which the primary building has been demolished (unknown number). All other deliverables and terms of the Scope of Services will remain as described and agreed to in the Professional Service Agreement # 8219, “Loomis Addition Survey.” IN WITNESS WHEREOF, the parties have executed this Amendment the day and year shown.
