In the event of a conflict between or ambiguity in the terms of the Services Agreement and this work order (including the attached forms) the Services Agreement shall control. The attached forms consisting of five (5) pages are hereby accepted and incorporated herein by this reference, and Notice to Proceed is hereby given. A, WINDSOR, COLORADO 80550 TEL. 970.674.3300 – FAX 970.674.3303 March 21, 2016 Mr. Matt Day City of Fort Collins Park Planning and Development PO Box 580 Fort Collins, CO 80522-0580 Maple Hill Park – Project Description and Scope of Services Dear Matt, Attached is the project scope and description: Description Interwest will work with your department and the BHA design team to prepare construction plans and bid documents for Maple Hill Park in Fort Collins. Larsen Structural Design will be a subconsultant to Interwest on this project. The park is approximately 7 acres. Scope of Services - Interwest Civil Engineering Plans – Interwest will prepare the required grading, erosion control, and utility plans as well as the drainage and erosion control report required for the project’s approval by the Box Elder Sanitation District, ELCO Water district, and The City of Fort Collins Development Review team. Interwest will also prepare trail plan and profiles for the approximately 1,100 LF 10’ wide multi-use trail that runs from the park to Thoreau Drive. The plans will be processed through the Basic Development Review process. BHA will provide fine grading in the park core area. Estimated Fee: $24,964 Irrigation Lateral Extension – Interwest will work with Hines Irrigation to prepare plan and profiles for the extension of the Baker Irrigation Lateral under street right-of-way to the park site. The extension will require crossing two intersections in public right-of-way which will be detailed by Interwest. Required surveying and potholing will be provided by the City. Estimated Fee: $3,056 Plan Coordination – Interwest will work with the BHA design team to prepare final bid documents for the park. Interwest will assist BHA in preparation of the bid tab, cost estimates, and project specifications for civil related improvements. It is assumed that the majority of civil related specifications will be covered through notes and details on the construction plans. Estimated Fee: $5,940 SWMP – Interwest will prepare an erosion control plan and stormwater management plan for the initial application to the State for the project. Estimated Fee: $2,010 Meetings & Coordination – Interwest will meet with the BHA design team and City Staff as necessary to complete the approval of the project. Estimated Fee: $4,006 Reimbursables– for prints, mileage, and deliveries, etc… Estimated Fee: $1,000 Interwest Fee: $40,976 Scope of Services – Larsen Structural Design (Larsen) Larsen will provide the design of structural elements for the following site structures: o Light Pole Foundations (2 different types assumed) o Seat Walls and foundations (2 different types assumed) o Retaining walls and foundations (2 different types assumed) o Miscellaneous footings for site amenities o Miscellaneous concrete detailing for steps in flatwork, curbs, etc.  Structural information to be red-marked on BHA’s drawings. BHA will perform all drafting services. LARSEN to stamp BHA’s drawings as necessary.  Review and edit the structural sections of the specifications.  Larsen’s services will be consistent with the “Recommended Standard of Practice for Engineering Services in Colorado” as developed by the Structural Engineers Association of Colorado. Larsen will provide the structural elements design for three free standing structures on the site: the irrigation pump house, rest room, and a picnic shelter. Structures are anticipated to be conventionally framed structural steel structures utilizing concrete masonry bearing walls. STRUCTURAL DESIGN SERVICES:  Provide full structural services for the project. Where applicable and noted above, we will “red- mark” information on the architectural plans and details (i.e. stairs, architectural metals, etc.). Our services will be consistent with the “Recommended Standard of Practice for Engineering Services in Colorado” as developed by the Structural Engineers Association of Colorado.  We will produce the contract documents using Revit Structure or AutoCAD (at our discretion) employing the client’s base model but utilizing our own drafting standards.  Value Engineering shall be an ongoing process during the Schematic Design and Design Development Phases. Services to revise drawings due to Value Engineering decisions made after Construction Documents are underway may be compensated as Additional Services depending on the scope of the changes.,  Fee is based on Construction Documents being issued in a single package for building permits and construction (no “footing and foundation” or multiple bid packages). If multiple bid packages are required, the Base Fee shall be increased twenty percent. STRUCTURAL CONSTRUCTION SERVICES:  Assist the general contractor in design related questions such as interpretations/clarifications of the contract drawings and responding to Requests for Information (RFIs).  Review shop drawings and submittals for structural compliance. During shop drawing review, LARSEN will return one marked-up reproducible set of shop drawings (electronic or otherwise) to the Architect for use by the general contractor.  Construction Coordination including site visits as required during the construction phase of the project. Site visits shall be made on a periodic basis to review the work in progress. Special Periodic Inspection, should it be required by the Contract Documents or at the request of the owner or building official, and inspection of each and every concrete pour is not within the scope of this Base Fee and shall be performed by an Independent Testing Agency hired by the owner. No surveying services are included – all topographic survey, construction survey, platting and legal descriptions will be provided by the City. 2. No changes to the adjacent public streets are anticipated. 3. No formal public outreach as it relates to the park is included. 4. Environmental, structural, traffic, electrical, and geotechnical engineering provided by others. 5. Construction Administration, Construction Management, Record Drawings, and Drainage/Site Certification are not included (unless noted otherwise).
