Taft Avenue Loveland, CO 80537 RE: Renewal, Independent Contractor Services Agreement Dear Mr. Shelley: The City of Fort Collins wishes to extend the agreement term for the above captioned proposal per the existing terms and conditions and the following: 1) The term will be extended for one (1) additional year, January 1, 2016 through December 31, 2016. 2) The fee for 2016 is forty seven thousand, three hundred ninety two dollars ($47,392). If the renewal is acceptable to your firm, please sign this letter in the space provided and include a current copy of insurance certificate naming the City as an additional insured for General Liability within the next fifteen (15) days. If this extension is not agreeable with your firm, we ask that you send us a written notice stating that you do not wish to renew the contract and state the reason for non-renewal. As discussed at the Forum’s December 15th board meeting, $37,392 is the cost associated with the Joint Funding Agreement (JFA) with the U.S. Geological Survey (USGS) and $10,000 is for Forum annual operations. The enclosed cost distribution spreadsheet provides operational cost breakdowns from USGS and for each of our major contributors based on a pro-rated formula for site ownership. Total costs to the city went up slightly, $1,210, in 2016. This is a 3% increase in the total JFA program from 2015. On behalf of the Forum, I would like to thank you and the City of Fort Collins for your continued in-kind laboratory support of the Forum. Your support and funding allows us to continue to monitor TOC and chlorophyll a and accomplish our mission in the Big Thompson River through collaborative monitoring, assessment, outreach, and restoration projects. With your funding, our major operating programs continue to remain strong and we look forward to 2016. The City of Fort Collins has been one of our most loyal supporters and it is making a difference in our ability to protect and improve water quality in the lakes, reservoirs, rivers and streams in the Big Thompson Watershed.
