In response to this bid, please submit the following: Page 3 of the bid, Page 5
of the bid (Bid Acknowledgement page) and the Transformer Bid Loss Data
Sheet. All may be submitted electronically in Microsoft Word or PDF format
and e-mailed to: purchasing@fcgov.com.

regarding this addendum.
RECEIPT OF THIS ADDENDUM MUST BE ACKNOWLEDGED BY A WRITTEN
