DATED: June 17, 2015 Work Order No. 22 Purchase Order No. Project title: Design Services for Timberline Road over Poudre River Expansion Joint Replacement Project Commencement Date: May 1, 2015 Completion Date: November 30, 2015 Maximum Fee: $55,637.17 PROJECT DESCRIPTION The purpose of this project is to provide final design services, including construction documents, for the replacement of the bridge expansion joints for the bridge carrying Timberline Road over the Poudre River in Fort Collins, Colorado. The existing bridge (1997) is three spans, 343 feet long and 56 feet wide. There are two expansion joints on the bridge, one at each abutment. Both expansion joints are 0"-4" strip seal expansion joints located between the end of the approach slab and the stem of the sleeper slab. The joints are aligned normal to the bridge centerline and extend full width, including through the sidewalks. Recently, the end dames which hold the expansion device rails have experienced deterioration resulting in portions of the steel expansion anchors to break out. As a result, the CITY plans to replace both the expansion joints through a rehabilitation effort. This scope of work defines the effort required to provide bid documents for the replacement of the expansion joints. Note, the CITY will perform an immediate temporary repair to the joints until such a time that these permanent repairs can be executed. SCOPE OF SERVICES: The scope of services for this project is to perform design services and prepare construction documents for the replacement of both expansion joints on the Timberline Road Bridge over the Poudre River. Work elements include adjustment to the joint elevations, asphalt surfacing, and concrete curbs at the bridge. The joints will be replaced with 0"-4" strip seal expansion joints (type to be consistent with pre -approved joints as defined by the Colorado Department of Transportation). Exhibit A - Work Order Form Design Services for Timberline Road over Poudre River Expansion Joint Replacement Project BAKER will provide design services including: project management, structural design, traffic phasing, cost estimating, and bidding assistance. All work will be based upon information determined from the original construction plans for the bridge and a survey to be provided by the CITY. Services not included in this scope of work include, but are not limited to: utility coordination, topographical survey, environmental coordination and permitting, right-of-way services, roadway and post design services. Additionally it is assumed that there will not be any impacts to the Poudre River and therefore this scope does not include any work related to a CLOMR or LOMR. Design criteria, standard details, and specifications shall first follow CDOT guidelines and secondly follow CITY guidelines. Final construction documents shall be consistent with CDOT requirements. The design process will only include one review by the CITY. The final product for this scope is a complete construction bid package (plans and estimate) for the expansion joint replacement of the Timberline Road Bridge over the Poudre River. The CITY will coordinate all contracting and logistical elements directly with the Contractor. Construction documents will be prepared in accordance with this contracting method. I. PROJECT ADMINISTRATION Upon notice to proceed, BAKER will perform project administration duties throughout the length of the contract. A. Invoices will be provided to the CITY on a monthly basis and will include a billing summary and a progress report for work performed during the billing period. B. BAKER will develop and maintain a project schedule. C. All documents will receive a quality assurance and quality control (QA/QC) review prior to submittal. D. The following meetings are anticipated: • One Pre -Design Meeting (Fort Collins) • One Traffic Control Review Meeting (Fort Collins) • One Design Review Meeting (Fort Collins) • One Pre -Construction Meeting (Fort Collins) E. BAKER will prepare documentation of meetings and telephone conversations and provide them to the CITY for review and recording keeping. II. FINAL DESIGN The final design phase includes preparation construction document preparation for the project. Work elements include: A. Surveying / Survey Mapping — BAKER will submit a survey request to the CITY and the CITY will survey the existing ground conditions and other relevant topography. Utilities —The CITY will coordinate with the utility companies affected by the project. BAKER is not designing or detailing any utility relocations. C. Geotechnical Investigation — No geotechnical investigation will be performed. D. Easements / Right of Way — The CITY will evaluate existing ROW / ownership mapping. BAKER will coordinate with the CITY to determine limits of ROW and easement needs. No legal descriptions or exhibits will be provided within this scope. E. Environmental Documentation — No environmental process will be performed. F. Civil / Roadway Design — BAKER will perform a drainage analysis necessary for all impacted facilities. G. Traffic Control Plan — BAKER will prepare a construction phasing plan that is consistent with traffic, geometric, and contractor work zone constraints associated with the bridge expansion joint replacement. The phasing plan will be submitted to the CITY for review and approval. It is anticipated that a two -phased construction approach will be developed. The CITY will perform traffic control services in accordance with the phased construction activities. H. Structural Design — BAKER shall prepare engineering design calculations as necessary. A construction field package will not be included in this scope. I. Construction Plans — BAKER shall prepare construction plans for the full scope of the project. The plans shall be QC reviewed prior to submittal to the CITY. Construction plans shall be prepared in the Microstation format. J. Project Specifications — BAKER will prepare project special provisions. The CITY will complete the boiler plate that includes the General Conditions. Specifications shall follow CITY specifications where applicable, and CDOT 2011 specifications otherwise. K. Construction Cost Estimate — BAKER will prepare quantities and an engineer's estimate of probable construction costs for all work items anticipated on the project. III. ADVERTISEMENT The advertisement design phase includes revisions to the final design and construction documents in response to comments provided by the CITY from the design review. Work elements include: A. Final Design — BAKER shall revise the engineering design according to comments from the CITY. All documents signed and sealed. B. Contract Documents — BAKER shall revise construction plans and project special provisions according to comments from the CITY. C. Utility Coordination & Design — The CITY shall contact utility companies to verify the progress of relocations. Construction Cost Estimate — BAKER will prepare quantities and an engineer's estimate of probable construction costs for all work items anticipated on the project. E. Assistance with Project Bidding - It is anticipated that a traditional bidding process will not be followed for this project. BAKER will attend a pre -construction meeting with the CITY and Contractor. F. Preparation of revised "For Construction" Plan Set — Prepare a final plan set and contract document package that incorporates any revisions required during the pre -construction meeting. Plans shall be marked "FOR CONSTRUCTION, (DATE)". All documents signed and sealed. IV. POST — DESIGN SERVICES A. Construction Administration Assistance — Post -Design services are excluded from this scope of work. If requested, BAKER can provide construction administration and design support services under a separate contract. Construction administration effort includes: RFI responses, shop drawing review, and assistance in the field. V. REFERENCES AND DESIGN CRITERIA The following references and design criteria shall be used throughout all phases of the project: A. All design shall follow the Larimer County Urban Area Streets Standards. B. All structural design shall follow the load and resistance factor design (LRFD) methodology. Design shall be in accordance with the AASHTO LRFD Bridge Design Specifications, 6th Edition. In addition, the design shall follow all CDOT technical documents, including: Bridge Design Manual, Bridge Detailing Manual, Load Rating Manual, and Technical Memorandums. Expansion joint details shall be prepared in accordance with applicable CDOT bridge worksheets. C. All roadway design shall follow the AASHTO Roadside Design Guide, 4th Edition and AASHTO Policy on Geometric Design of Highways and Streets, 6th Edition. D. Construction specifications will be in accordance with the Colorado Department of Transportation 2011 Standard Specifications for Road and Bridge Construction and related special provisions. VI. DELIVERABLES The following is an estimate of the deliverables required for this project. Refer to the detailed discussions above for further detail. All copies shall be submitted in hard copy and .pdf format; plans are 11x17, all others S'/2 xl 1, except as noted otherwise.
