Auxiliary Documents: Could you confirm what financial data is being requested? Is this part of point #3 within Exhibit “B”? Answer: Financial data should detail financial assets that are relative to the operation of this concession. 2. Could you confirm that when we submit our proposal, that these items are the proper ones we should be emailing to the purchasing@fcgov.com email address? - Qualification Statement (Exhibit “B”) - Auxiliary Documents. - References. - Completion of Proposal Form (Exhibit “C”). - Exhibit “ “ Disclosure to Employee/ Volunteer Regarding Procurement of a Consumer Report. - Exhibit “ “ Affidavit Pursuant to C.R.S. 24-76.5-103 Answer: The following are documents required to be submitted for this RFP: completion of Qualification Statement (Exhibit “B”), and all auxiliary documents annotated on Exhibit B, and completion of the Proposal Form, (Exhibit “C”). Exhibit “ “ Disclosure to Employee/Volunteer Regarding Procurement of a Consumer Report and Exhibit “ “ Affidavit Pursuant to C.R.S. 24-76.5-103 are not required to be submitted with the proposal. Please refer to Section III Proposal Requirements in the RFP for further information. 3. Our understanding is that this proposal should be contained within 5 pages total or less.
