Project Description: Transition existing construction permits to long term permanent permits for Port of
Entry and Shields Pits. This includes analysis and close out of existing permits
design of temporary and permanent BMP measures and CDPHE SWMP permits to
control storm event discharge during vegetative growth and thereafter.

Professional agrees to perform the services
identified above and on the attached forms in
accordance with the terms and conditions
contained herein and in the Professional Services
Agreement between the parties. In the event of a
conflict between or ambiguity in the terms of the
Professional Services Agreement and this work
order (including the attached forms) the
Professional Services Agreement shall control.
The attached forms consisting of Three (3) pages
are hereby accepted and incorporated herein, by
this reference, and Notice to Proceed is hereby
given.

This scope of work letter follows on our recent discussions regarding your need to transition the existing pit
commercial uselconstruction permits to long term permanent pond use permits for both the Port of Entry Pit and
Shields Pond. We understand the scope to involve analysis and closeout of the existing permits, design of
temporary and permanent BMP measures and CDPHE SWMP permit filing to control storm event discharge during
vegetative growth and in a permanent manner thereafter. Our itemized scope of work proposal is attached to this
letter.
Ditesco proposes to complete the above and attached scope of services for a not to exceed cost of $5,410.00. We
plan to complete this work effort over the month of May 2015.
