2. Description of Change:
Mobilize a technician to plug and abandon four monitoring wells (MW-5, MW-7, MW-8 and AS-1) per
Colorado State Engineer standards. The wells are circled on the attached Site Map.
• Saw-cut the concrete around all four manways, add dowels, and patch concrete over each core to
prevent concrete settling.
• Prepare and submit Well Abandonment Reports to the Colorado Department of Natural
Resources – Division of Water Resources (DWR) as required for all permitted wells.
• Prepare and submit a Site Decommissioning Report (SDR) to OPS.

Dear Ms. Henggeler:
This correspondence presents CGRS, Inc.’s (CGRS’) scope of work and cost estimate for decommissioning all site
monitoring wells. The event is anticipated to achieve Tier-1 Closure following the fourth quarter 2020 monitoring
event, and the wells will be decommissioned by the end of the second quarter 2021. The scope of work detailed
below will be performed as a change order to the current PO# 9192222, which appears to have enough budget
remaining to cover the costs of these activities. The requested change order would include verbiage for the scope
of work below, and an expiration extension to June 30, 2021 in order to complete the additional work.
Scope of Work
The proposed scope of work will include the following:
Mobilize a technician to plug and abandon four monitoring wells (MW-5, MW-7, MW-8 and AS-1) per
Colorado State Engineer standards. The wells are circled on the attached Site Map.
Saw-cut the concrete around all four manways, add dowels, and patch concrete over each core to prevent
concrete settling.
Prepare and submit Well Abandonment Reports to the Colorado Department of Natural Resources –
Division of Water Resources (DWR) as required for all permitted wells.
Prepare and submit a Site Decommissioning Report (SDR) to OPS.
Limitations
The work will be performed based upon the information provided to CGRS and on our observations made at the
time of field activities. Given the inherent limitations of environmental assessment work, CGRS does not guarantee
that latent or undiscovered conditions will not become evident in the future. No other warranties, representations,
or certifications are made.
Petroleum impacts in the subsurface are regulated by Colorado OPS and it is at their sole discretion to regulate,
enforce, and interpret petroleum storage tank regulations. CGRS provides consultation services concerning state

and federal regulations and interaction with government agencies but makes no warranties, expressed or implied,
concerning decisions and outcomes determined by State or Federal regulatory agencies.
Project Compensation
Based upon the scope of work described in this proposal, the budget remaining for PO# 9192222 appears
to be sufficient for the estimated costs to complete the site decommissioning and reporting. All costs for
this proposal were completed according to the Colorado OPS Reasonable Cost Guidelines (RCGs), which are
specific rates set by the State. The requested change order would include verbiage for the scope of work detailed
above, and an expiration extension to June 30, 2021 in order to complete the additional work.
