MAXIMUM FEE: (time and reimbursable direct costs): $18628.19
PROJECT DESCRIPTION/SCOPE OF SERVICES:
Furnish and install a new Maxton valve to replace the obsolete valve. Replace hydraulic
packing on both cylinders to improve the reliability of the lift. Perform adjustments and
return the unit to normal working condition.
Service Provider agrees to perform the services identified above and on the attached forms in
accordance with the terms and conditions contained herein and in the Master Agreement between
the parties. In the event of a conflict between or ambiguity in the terms of the Master Agreement
and this Work Order (including the attached forms) the Master Agreement shall control.
The attached forms consisting of four (4) pages are hereby accepted and
incorporated herein, by this reference, and Notice to Proceed is hereby given after all parties
have signed this document.

Description of Work
Valve and Packing Replacement
As your maintenance provider it is our responsibility to provide you with safe, reliable elevators and lifts for your
facility in order to better serve you and your tenants. The current valve on the stage lift is obsolete; and towards
the end of its useful life. We will furnish and install a new Maxton valve to replace the obsolete valve. We will
also replace the hydraulic packing on both cylinders improving the reliability of the lift. Once installation is
complete, KONE will perform any necessary adjustments and return the unit to normal working condition.
PLEASE NOTE: Current lead time on material is 4-6 weeks. Work will take approximately three (3) days to
complete. Work will be completed during normal working hours.
Price
Our total price to perform the above-mentioned work amounts to: $18,628.19, including applicable taxes.
Labor: $563.81 per team hour * 24 team hours = $13,531.44
Material: $5,096.75
Total: $18,628.19
