Dallas, TX 75229 RE: Contract Renewal, 8813 Custodial Services Dear Mr. Jae Song: The City of Fort Collins wishes to extend the agreement term for the above captioned agreement per the existing terms and conditions and the following: 1) The term for the following Custodial Packages will be extended for one (1) additional year, May 21, 2020 through May 20, 2021. a. Package 1 – 212 N. Mason/ 222 Laporte/ 281 N. College b. Package 2 – City Hall and Operation Services Building (300 Laporte) c. Package 6 – Nix Farm/ Streets/ Traffic d. Package 9 – Parks e. Package 13 – Fleet/Warehouse 2) The term for the following Custodial Packages will be extended for the period May 21, 2020 through July 31, 2020. a. Package 5 – Libraries b. Package 10 – Water Treatment Facility c. Package 11 – Utilities Service Center d. Package 12 – Utilities If the renewal is acceptable to your firm, please sign this letter in the space provided and include a current copy of insurance certificate naming the City as an additional insured for General and Automotive Liability within the next ten (10) days. If this extension is not agreeable with your firm, we ask that you send us a written notice stating that you do not wish to renew the contract and state the reason for non-renewal.
