RE: Renewal, Miscellaneous Agreement - Diversity Solutions Group
Dear Vendor:
The City of Fort Collins wishes to extend the agreement term for the above captioned proposal
per the existing terms and conditions and the following:
1) The term will be extended for one (1) additional year, April 28, 2020 through April 27,
2021.
If the renewal is acceptable to your firm, please sign this letter in the space provided and
include a current copy of insurance certificate naming the City as an additional insured
for General Liability within the next fifteen (15) days.
If this extension is not agreeable with your firm, we ask that you send us a written notice stating
that you do not wish to renew the contract and state the reason for non-renewal.

matter.

Dear Mr. Paul,
Diversity Solutions Group, LLC intends to continue providing consulting/training services to the
City of Fort Collins in 2020. We have secured the required co-insurance for the City, which is
due for renewal in March 2020. I have requested that the Certificate be expedited and sent to
you as soon as possible.
I apologize for this lag in time. Please look for the Certificate coming to you directly from our
insurer, Tad Borrett at Security Insurance Group. Let me know if you have any questions.
