III. A) eScreen
AccuTest of Northern Colorado is an Alere/eScreen preferred provider and our eScreen location ID number
is 38071. We already provide drug testing services for the City of Fort Collins and have the necessary
testing paperwork (DOT FMCSA, DOT FTA, and Non-Dot CCF’s ) specimen collection kits, and necessary
equipment in stock.
III B) Qualifications
All of our collectors are certified compliant with DOT standards for drug specimen collections and
breathalyzer testing. Our in-house lab and MRO are the same DOT approved providers that the City of Fort
Collins currently contracts with. (Alere Laboratories and Dr. Stephen Kracht as our MRO). Our
breathalyzers are the DOT approved Drager Model 6820.
We have an office location for any testing during business hours and provide 24/7/365 mobile on-site
testing at your locations.

III E) General description
Accu-Test of Northern Colorado LLC is a Woman/Minority/LGBT owned company. We have one office
location in Loveland at 2529 N. Lincoln Ave. There is a large waiting room with many chairs, fish tanks,
magazines, and a television. Water and coffee are available for donors. The testing bathroom meets all
DOT requirements and there is a locked cabinet for donors to secure their possessions while testing. The
eScreen computer is located in the back testing room for donor privacy. All testing paperwork is scanned
into our computer for indefinite record storage. The computer is password protected and is backed up to a
protected external hard drive and to a protected off-site cloud service. Any hard copies of testing
information are secured in locked file cabinets in a locked office that is only accessible to employees. All
testing specimens are kept in an “employee only” room until they are picked up by a Fed Ex courier daily
between 4-5pm.
