In the event of a conflict between or ambiguity in the terms of the Master Agreement and this Work Order (including the attached forms) the Master Agreement shall control. The attached forms consisting of two (2) pages are hereby accepted and incorporated herein, by this reference, and Notice to Proceed is hereby given after all parties have signed this document. SERVICE PROVIDER: Date: 3-18-19 Michael Oberlander, Sr. PM ACCEPTANCE: Date: Name, Project Manager REVIEWED: Date: Name, Buyer or Senior Buyer ACCEPTANCE: Date: Gerry Paul, Purchasing Director (if greater than $60,000) March 18, 2019 - REVISED December 5, 2019 Mr. Tracy Dyer City of Fort Collins - Engineering Suniga Construction Services and Staking – Add Request #1 Project Description and Scope of Services Dear Tracy, Description Suniga Road construction will be starting in April. The City has asked that Interwest, BHA, and Hines Irrigation be available for construction services, and that King Surveyors provide construction staking for the project. It is assumed that some field visits will be required, but the fee below does not include attendance at weekly meetings. Construction services will include material and shop drawing submittal review, field visits at the City’s request, and punch walks. BHA and Hines’ fees below have been increased from the original agreement because the project was more complicated than originally scoped (coordination with neighboring parcels and the design east of the roadway project to Redwood). BHA will also need to be involved in both the roadway construction and the landscape construction with two different contractors because of the split of the project into two separate bids. Additional fees are being requested for the following additional services provided: 1. Restaking by King that where additional services were not immediately requested. 2. Additional design for the landscaping by BHA because of some comments that were received out of sequence. This added design was done after the original design contract was already closed out. BHA provided 4 sets of drawings in the weeks prior to bidding to address various department comments. 3. Additional time for Interwest to coordinate the landscape and irrigation bid set. 4. Additional time during construction for Interwest to review and prepare exhibits related to utility installations and areas where the waterline was not built per design. Construction Services and Staking Fee Summary: BHA Design - $3,400 $6,400 Hines Irrigation - $1,600 $1,600 Interwest Consulting $9,700 $14,200 King Surveyors $26,450 $28,950 Total $41,150 $51,150 BHA and Hines will continue to bill to the existing design work order until award of the landscape and irrigation package.
