Official Purchasing Document Last updated 10/2017 SERVICES AGREEMENT THIS AGREEMENT made and entered into the day and year set forth below by and between THE CITY OF FORT COLLINS, COLORADO, a Municipal Corporation, hereinafter referred to as the "City" and NATIONAL CENTER FOR CRAFTSMANSHIP, hereinafter referred to as "Service Provider". WITNESSETH: In consideration of the mutual covenants and obligations herein expressed, it is agreed by and between the parties hereto as follows: 1. Scope of Services. The Service Provider agrees to provide services in accordance with the scope of services attached hereto as Exhibit A, consisting of four (4) pages and incorporated herein by this reference. Irrespective of references in Exhibit A to certain named third parties, Service Provider shall be solely responsible for performance of all duties hereunder. 2. The Work Schedule. The services to be performed pursuant to this Agreement shall be performed in accordance with the Work Schedule attached hereto as Exhibit B, consisting of one (1) page, and incorporated herein by this reference. 3. Time of Commencement and Completion of Services. The services to be performed pursuant to this Agreement shall commence no later than October 14, 2019. Services shall be completed no later than November 30, 2019. Time is of the essence. Any extensions of the time limit set forth above must be agreed upon in a writing signed by the parties. 4. Delay. If either party is prevented in whole or in part from performing its obligations by unforeseeable causes beyond its reasonable control and without its fault or negligence, then the party so prevented shall be excused from whatever performance is prevented by such cause. To the extent that the performance is actually prevented, the Service Provider must provide written notice to the City of such condition within fifteen (15) days from the onset of such condition. 5. Early Termination by City/Notice. Notwithstanding the time periods contained herein, the City may terminate this Agreement at any time without cause by providing written notice of termination to the Service Provider. Such notice shall be delivered at least fifteen (15) days prior to the termination date contained in said notice unless otherwise agreed in writing by the parties. Such payment shall be the Service Provider's sole right and remedy for such termination. 6. Contract Sum. The City shall pay the Service Provider for the performance of this Contract, subject to additions and deletions provided herein, Nine Thousand Four Hundred Dollars ($9,400) as per the attached Exhibit A, consisting of four (4) pages. Payment shall be made by the City Net 30 days from the date of the invoice and upon acceptance of the work by the City and upon the Service Provider furnishing satisfactory evidence of payment of all wages, taxes, supplies and materials, and other costs incurred in connection with the performance of such work. 7. City Representative. The City will designate, prior to commencement of the work, its representative who shall make, within the scope of his or her authority, all necessary and proper decisions with reference to the services provided under this agreement. All requests concerning this agreement shall be directed to the City Representative. 8. Independent Service provider. The services to be performed by Service Provider are those of an independent service provider and not of an employee of the City of Fort Collins. The City shall not be responsible for withholding any portion of Service Provider's compensation hereunder for the payment of FICA, Workmen's Compensation or other taxes or benefits or for any other purpose. 9. Subcontractors. Service Provider may not subcontract any of the Work set forth in the Exhibit A, Statement of Work without the prior written consent of the city, which shall not be unreasonably withheld. If any of the Work is subcontracted hereunder (with the consent of the City), then the following provisions shall apply: (a) the subcontractor must be a reputable, qualified firm with an established record of successful performance in its respective trade performing identical or substantially similar work, (b) the subcontractor will be required to comply with all applicable terms of this Agreement, (c) the subcontract will not create any contractual relationship between any such subcontractor and the City, nor will it obligate the City to pay or see to the payment of any subcontractor, and (d) the work of the subcontractor will be subject to inspection by the City to the same extent as the work of the Service Provider. 10. Personal Services. It is understood that the City enters into the Agreement based on the special abilities of the Service Provider and that this Agreement shall be considered as an agreement for personal services. Accordingly, the Service Provider shall neither assign any responsibilities nor delegate any duties arising under the Agreement without the prior written consent of the City. 11. Acceptance Not Waiver. The City's approval or acceptance of, or payment for any of the services shall not be construed to operate as a waiver of any rights or benefits provided to MISC SA - Archery Range Garage Deconstruction Page 2 of 43 DocuSign Envelope ID: C3A8ECD2-3B48-4BB2-9F45-670A94D65E6C Official Purchasing Document Last updated 10/2017 the City under this Agreement or cause of action arising out of performance of this Agreement. 12. Warranty. a. Service Provider warrants that all work performed hereunder shall be performed with the highest degree of competence and care in accordance with accepted standards for work of a similar nature. b. Unless otherwise provided in the Agreement, all materials and equipment incorporated into any work shall be new and, where not specified, of the most suitable grade of their respective kinds for their intended use, and all workmanship shall be acceptable to City. c. Service Provider warrants all equipment, materials, labor and other work, provided under this Agreement, except City-furnished materials, equipment and labor, against defects and nonconformances in design, materials and workmanship/workwomanship for a period beginning with the start of the work and ending twelve (12) months from and after final acceptance under the Agreement, regardless whether the same were furnished or performed by Service Provider or by any of its subcontractors of any tier. Upon receipt of written notice from City of any such defect or nonconformances, the affected item or part thereof shall be redesigned, repaired or replaced by Service Provider in a manner and at a time acceptable to City. 13. Default. Each and every term and condition hereof shall be deemed to be a material element of this Agreement. In the event either party should fail or refuse to perform according to the terms of this agreement, such party may be declared in default thereof. 14. Remedies. In the event a party has been declared in default, such defaulting party shall be allowed a period of ten (10) days within which to cure said default. In the event the default remains uncorrected, the party declaring default may elect to (a) terminate the Agreement and seek damages; (b) treat the Agreement as continuing and require specific performance; or (c) avail himself of any other remedy at law or equity. If the non-defaulting party commences legal or equitable actions against the defaulting party, the defaulting party shall be liable to the non-defaulting party for the non-defaulting party's reasonable attorney fees and costs incurred because of the default. 15. Binding Effect. This writing, together with the exhibits hereto, constitutes the entire agreement between the parties and shall be binding upon said parties, their officers, employees, agents and assigns and shall inure to the benefit of the respective survivors, heirs, personal representatives, successors and assigns of said parties. 16. Indemnity/Insurance. a. The Service Provider agrees to indemnify and save harmless the City, its officers, agents and employees against and from any and all actions, suits, claims, demands or liability of any character whatsoever brought or asserted for injuries to or death of any person or persons, or damages to property arising out of, result from or occurring in connection MISC SA - Archery Range Garage Deconstruction Page 3 of 43 DocuSign Envelope ID: C3A8ECD2-3B48-4BB2-9F45-670A94D65E6C Official Purchasing Document Last updated 10/2017 with the performance of any service hereunder. b. The Service Provider shall take all necessary precautions in performing the work hereunder to prevent injury to persons and property. c. Without limiting any of the Service Provider's obligations hereunder, the Service Provider shall provide and maintain insurance coverage naming the City as an additional insured under this Agreement of the type and with the limits specified within Exhibit C, consisting of one (1) page, attached hereto and incorporated herein by this reference. The Service Provider before commencing services hereunder, shall deliver to the City's Purchasing Director, P. O. Box 580, Fort Collins, Colorado 80522, one copy of a certificate evidencing the insurance coverage required from an insurance company acceptable to the City. 17. Entire Agreement. This Agreement, along with all Exhibits and other documents incorporated herein, shall constitute the entire Agreement of the parties. Covenants or representations not contained in this Agreement shall not be binding on the parties. 18. Law/Severability. The laws of the State of Colorado shall govern the construction interpretation, execution and enforcement of this Agreement. In the event any provision of this Agreement shall be held invalid or unenforceable by any court of competent jurisdiction, such holding shall not invalidate or render unenforceable any other provision of this Agreement. 19. Prohibition Against Employing Illegal Aliens. Pursuant to Section 8-17.5-101, C.R.S., et. seq., Service Provider represents and agrees that: a. As of the date of this Agreement: 1. Service Provider does not knowingly employ or contract with an illegal alien who will perform work under this Agreement; and 2. Service Provider will participate in either the e-Verify program created in Public Law 208, 104th Congress, as amended, and expanded in Public Law 156, 108th Congress, as amended, administered by the United States Department of Homeland Security (the “e-Verify Program”) or the Department Program (the “Department Program”), an employment verification program established pursuant to Section 8- 17.5-102(5)(c) C.R.S. in order to confirm the employment eligibility of all newly hired employees to perform work under this Agreement. b. Service Provider shall not knowingly employ or contract with an illegal alien to perform work under this Agreement or knowingly enter into a contract with a subcontractor that knowingly employs or contracts with an illegal alien to perform work under this Agreement. c. Service Provider is prohibited from using the e-Verify Program or Department Program procedures to undertake pre-employment screening of job applicants while this Agreement is being performed. If Service Provider obtains actual knowledge that a subcontractor performing work under this Agreement knowingly employs or contracts with an illegal alien, Service Provider shall: 1. Notify such subcontractor and the City within three days that Service Provider has actual knowledge that the subcontractor is employing or contracting with an illegal alien; and 2. Terminate the subcontract with the subcontractor if within three days of receiving the notice required pursuant to this section the subcontractor does not cease employing or contracting with the illegal alien; except that Service Provider shall not terminate the contract with the subcontractor if during such three days the subcontractor provides information to establish that the subcontractor has not knowingly employed or contracted with an illegal alien. e. Service Provider shall comply with any reasonable request by the Colorado Department of Labor and Employment (the “Department”) made in the course of an investigation that the Department undertakes or is undertaking pursuant to the authority established in Subsection 8-17.5-102 (5), C.R.S. f. If Service Provider violates any provision of this Agreement pertaining to the duties imposed by Subsection 8-17.5-102, C.R.S. the City may terminate this Agreement. If this Agreement is so terminated, Service Provider shall be liable for actual and consequential damages to the City arising out of Service Provider’s violation of Subsection 8-17.5-102, C.R.S. g. The City will notify the Office of the Secretary of State if Service Provider violates this provision of this Agreement and the City terminates the Agreement for such breach. 20. Dust Control. The Service Provider shall abide by the City of Fort Collins “Dust Control and Prevention Manual,” which is available for public download at https://www.fcgov.com/airquality/pdf/dust-prevention-and-control-manual.pdf, and is incorporated herein by this reference. The City of Fort Collins has implemented this manual for all projects performed for the City of Fort Collins or located within the City of Fort Collins City limits. 21. Hazardous Material Assessment. The services to be performed pursuant to this Agreement shall be performed in accordance with The Hazardous Material Assessment for Demolition Report is set forth in Exhibit D, consisting of thirty (30) pages, attached hereto and incorporated herein by this reference. This material is not regulated and can be disposed of using traditional methods but cannot be recycled. All personnel must be protected from exposure to the Asbestos materials in accordance with the OSHA standard. o Price of any rentals, freight or surcharge fees for the building deconstruction are included in Lump Sum Base Bid. o Before final payment can be issued, the Waste Management plan forms must be completed. o The City of Fort Collins will: Provide the signature of the Certified Asbestos Inspector for the CDPHE demolition permit. Be responsible for any City of Fort Collins Stormwater department requirements that may arise as a result of permitting for the project. Provide the NCC with a copy of an Asbestos building survey report or certificate for the building showing that the building is free of asbestos containing materials (ACM). Any ACM’s or other hazardous materials that may be discovered by NCC that are not shown in the provided report will be safely removed and/or abated at the City of Fort Collins’ expense. Assure that all utilities, including gas, electric, water, etc. have been disconnected from the building prior to NCC beginning site activities. Allow NCC to maintain a portable storage trailer on site for the duration of the project. The Service Provider will provide, from insurance companies acceptable to the City, the insurance coverage designated hereinafter and pay all costs. Before commencing work under this bid, the Service Provider shall furnish the City with certificates of insurance showing the type, amount, class of operations covered, effective dates and date of expiration of policies, and containing substantially the following statement: “The insurance evidenced by this Certificate will not reduce coverage or limits and will not be cancelled, except after thirty (30) days written notice has been received by the City of Fort Collins.” In case of the breach of any provision of the Insurance Requirements, the City, at its option, may take out and maintain, at the expense of the Service Provider, such insurance as the City may deem proper and may deduct the cost of such insurance from any monies which may be due or become due the Service Provider under this Agreement. The City, its officers, agents and employees shall be named as additional insureds on the Service Provider 's general liability and automobile liability insurance policies for any claims arising out of work performed under this Agreement. 2. Insurance coverages shall be as follows: A. Workers' Compensation & Employer's Liability. The Service Provider shall maintain during the life of this Agreement for all of the Service Provider's employees engaged in work performed under this agreement: 1. Workers' Compensation insurance with statutory limits as required by Colorado law. 2. Employer's Liability insurance with limits of $100,000 per accident, $500,000 disease aggregate, and $100,000 disease each employee. B. Commercial General & Vehicle Liability. The Service Provider shall maintain during the life of this Agreement such commercial general liability and automobile liability insurance as will provide coverage for damage claims of personal injury, including accidental death, as well as for claims for property damage, which may arise directly or indirectly from the performance of work under this Agreement. Coverage for property damage shall be on a "broad form" basis. The amount of insurance for each coverage, Commercial General and Vehicle, shall not be less than $1,000,000 combined single limits for bodily injury and property damage. In the event any work is performed by a subcontractor, the Service Provider shall be responsible for any liability directly or indirectly arising out of the work performed under this Agreement by a subcontractor, which liability is not covered by the subcontractor's insurance. (LTE) has prepared this report on behalf of the City of Fort Collins (the City), summarizing the findings of a hazardous-material assessment for demolition of the Archery Range garage building located at 2825 Southeast Frontage Road in Fort Collins, Colorado (Figure 1). The hazardous material building assessment for demolition included the following tasks. • An asbestos-containing material (ACM) survey for demolition; and • Lead-containing building materials survey for potential waste generation. Homogeneous materials of suspect ACMs and painted surfaces suspect for lead were identified and documented within specific locations of the building. SAMPLING OBJECTIVES LTE understands this hazardous material building assessment was requested by the City to evaluate suspect building materials for the presence or absence of asbestos due to potential demolition activities to be completed. The United States Environmental Protection Agency (EPA) 40 Code of Federal Regulations (CFR) Part 61, National Emission Standards for Hazardous Air Pollutants (NESHAP), prohibits the release of asbestos fibers to the atmosphere during renovation or demolition activities. The asbestos NESHAP rule requires that potentially regulated asbestos-containing material (RACM) be identified, classified, and quantified prior to planned disturbance, renovation, or demolition activities. The purpose of collecting lead Toxicity Characteristic Leaching Procedure (TCLP) samples was to determine the likelihood of the potential waste stream generated during demolition activities to leach lead overtime at concentrations that exceed the TCLP lead standard of 5.0 milligrams per liter (mg/L) after being placed in a landfill. Page 2 SCOPE OF WORK The asbestos survey included a visual and physical assessment of materials subject to disturbance by demolition. Suspect ACM was sampled in conformance with the sampling protocols outlined in the EPA regulation 40 CFR 763.86, Asbestos Hazard Emergency Response Act (AHERA) and Colorado Department of Public Health and Environment (CDPHE) Regulation 8. Samples were delivered to an accredited laboratory for analysis by polarized light microscopy (PLM) per EPA methodology (EPA-600/R-93/116). A composite sample of representative building materials was collected to determine waste characterization and waste disposal using TCLP by EPA Test Method for Evaluating Solid Waste: Physical/ Chemical Methods Compendium (SW-846) Method 1311. The assessment was conducted on June 21, 2019, by Mr. Ryan Zernis, a Colorado-certified asbestos Building Inspector (CABI). Mr. Zernis is also a Colorado-certified Lead-Based Paint Inspector. Copies of Mr. Zernis’ state certifications and LTE Asbestos and Lead Consultant Firm Certification are included as Attachment 1. Site Description This hazardous material building assessment for demolition was conducted on the standalone garage building. The garage building is described as a 2-car, 1-story garage. According to Larimer County Land Assessor’s Office, the building is owned by the City, and no records related to the building’s construction were available. Visual Assessment LTE survey activities began with a visual observation of the building to identify functional spaces, homogeneous materials, and the building materials that would be impacted by demolition activities. Building materials and components observed in the building consisted of window glazing, wood studs and rafters, concrete masonry units (CMU), and roofing material. Physical Assessment A physical assessment of each suspect homogeneous ACM was conducted to assess the condition of the material as friable or non-friable. The EPA defines friable material as one that, when dry, can be crumbled, pulverized, or reduced to a powder by hand pressure. Friability was assessed by physically touching suspect ACM. LTE classified the current condition of the suspect ACM and the potential for damage to the suspect ACM. Page 3 ASBESTOS SAMPLE COLLECTION Bulk samples of suspect ACM were collected in conformance with AHERA and CDPHE Regulation 8 protocols. The samples were collected using wet methods as applicable to reduce the potential for a friable release. Samples were placed in sealable containers and labeled with unique sample identifications based on the material’s location. The asbestos sample locations are depicted on Figure 2. A total of seven bulk samples of suspect ACM were collected from the following materials. Miscellaneous Material: • Window glazing Surfacing Material: • CMU sealant The suspect ACM samples were submitted to Reservoirs Environmental, Inc. (Reservoirs) in Denver, Colorado, an independent laboratory that has successfully participated in the National Voluntary Laboratory Accreditation Program and is accredited by the American Industrial Hygiene Association. The bulk samples were submitted under strict chain-of-custody procedures for analysis by PLM. Visual estimation was used to obtain the percentage of asbestos present within the bulk samples. TCLP SAMPLE COLLECTION Based on the results of visual observations to determine the different painted surfaces, three samples were collected, and one composite sample was created. A composite sample of various painted surfaces and the matrix that they were adhered to from each building material were placed in a sample container. The samples were collected using wet methods as applicable to reduce the potential for a release. The sample was placed in a sealable container and labeled with a unique sample identification. The composite TCLP sample was collected from the following materials. • White-painted wood on garage door frame; • Tan-painted wood on garage door frame; • White-painted window glazing; and • White-painted CMU. REGULATORY OVERVIEW Asbestos The CDPHE Regulation 8, Hazardous Air Pollutants Control, Part B, Asbestos, and 5 Code of Colorado Regulations (CCR) 1001-10, Part B applies, in general, to buildings, facilities, and associated components. The CDPHE Air Pollution Control Division (APCD) is the implementing regulatory agency for NESHAP regulations. The Occupational Safety and Health Administration (OSHA) asbestos standard for the construction industry (29 CFR 1926.1101) regulates workplace exposure to asbestos. The OSHA standard requires that employee exposure to airborne asbestos fibers be maintained below 0.1 asbestos fibers per cubic centimeter (0.1 f/cc) of air for an 8-hour day. The OSHA standard classifies construction and maintenance activities, which could disturb ACM, and specifies work practices and precautions, which employers must follow when engaging in each class of regulated work. The asbestos NESHAP rule (40 CFR Part 61) regulates asbestos fiber emissions and asbestos waste disposal practices. It requires the identification and classification of existing building materials prior to renovation or demolition activities. Under NESHAP, asbestos-containing building materials are classified as either friable, Category I non-friable, or Category II non-friable ACM. Friable materials are those that, when dry, may be crumbled, pulverized, or reduced to powder by hand pressure. Category I non-friable ACM includes packings, gaskets, resilient flooring covering, and asphalt roofing products. Category II non-friable ACM are any materials other than Category I materials that contain more than 1 percent (%) asbestos. RACM is defined by CDPHE Regulation 8 as friable ACM and Category I and Category II non-friable ACM that are in poor condition and have become friable or will be subjected to sanding, grinding, cutting, or abrading. In addition, RACM includes Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized or reduced to powder by the forces expected to act on the material in the course of demolition or renovation operations. RACM must be removed prior to renovation or demolition activities. If the amount of RACM exceeds the trigger levels of 260 linear feet (LF), 160 square feet (SF), or the volume equivalent of one 55-gallon drum, the owner or operator must provide the CDPHE-APCD with written notification of planned removal activities at least 10 working days prior to the commencement of asbestos abatement activities. Removal of RACM must be conducted by a trained and appropriately licensed asbestos abatement contractor. Page 5 Lead Title IV of TSCA, as well as other authorities in the Residential Lead-Based Paint Hazard Reduction Act of 1992, direct EPA to regulate Lead-Based Paint (LBP) hazards. The EPA definition of LBP is paint or other surface coatings that contain lead at concentrations equal to or exceeding 1 milligram of lead per square centimeter (mg/cm2 ) of paint or greater or paint that contains lead greater than 0.5% lead by weight or 5,000 parts per million (ppm) by weight. In addition, the OSHA Regulation 29 CFR 1910.1025 outlines health and safety for workers who have the potential to disturb lead-containing paint or LBP. The OSHA standard requires that employee exposure to airborne lead be maintained below 50 micrograms per cubic meter (µg/m3). The TCLP is designed to determine the mobility of both organic and inorganic analytes present in liquid, solid, and multiphasic wastes if disposed of in a sanitary landfill. The TCLP analysis is used to determine if a constituent in an amount equal to or exceeding the concentrations specified in 40 CFR Part 261 meet the definition of EPA Toxicity in Resource Conservation Recovery Act (RCRA) (40 CFR Part 261). The regulatory allowable level for lead toxicity characteristic as listed in 40 CFR 268.40 Table 7-1 defines the maximum lead contaminant concentration of 5.0 mg/L. FINDINGS Asbestos Results of the visual and physical assessment, sample descriptions, and laboratory analytical results are summarized in Table 1. The laboratory analytical reports are included as Attachment 2, and photographs of samples are presented as Attachment 3. Based on the results of the laboratory analysis, no sample or identified material contained asbestos fibers greater than 1%. Laboratory analytical results for suspect lead-containing paints and matrices indicated that the TCLP sample did not contain a lead concentration that exceeded the maximum contaminant concentration of 5.0 mg/L. The leachable lead concentration in sample 2825-TCLP01 was 0.34 mg/L. The composite sample that was collected and submitted for laboratory analysis is identified in the lead TCLP sample description with the laboratory analytical result in Table 2. The laboratory analytical report is included as Attachment 2. CONCLUSIONS AND RECOMMENDATIONS LTE did not sample tar-impregnated roofing materials, shingles, tar (mastic), and/or roof sealants as the roof shingle materials that were observed appeared to be in good condition and, as noted by the City, have been recently replaced (within 10 years). The possibility of uncovering additional tar/asphaltic roofing materials may occur during demolition. However, CDPHE-APCD Regulation 8, Subpart B – Asbestos Control, asphaltic materials including tar-impregnated roofing felts, asphalt roofing tiles, roofing asphalt, and roofing mastics that are non-friable and that will remain non-friable during abatement are exempt from the regulation. Further tar/asphaltic- impregnated roofing felts, asphalt roofing tiles, roofing asphalts, and roofing mastics that are non-friable and in good condition may remain in a building during demolition as long as normal demolition practices are used (i.e., no burning or implosion, etc.). LTE has assumed that additional tar/asphaltic-impregnated roofing felts, asphalt roofing tiles, roofing asphalts, and roofing mastics that maybe encountered are ACM but non-friable and in good condition. LTE is providing the following recommendations. • Laboratory analytical results indicate that the identified window glazing, and CMU sealant contain trace amounts of asbestos at concentrations less than 1%. Page 7 not regulated by EPA and CDPHE-APCD, OSHA regulates workplace exposure to asbestos. All applicable work practices per 29 CFR 1926.1101 must be implemented when demolition activities are completed. • Prior to demolition of the building, a CDPHE-APCD Demolition Notification Application Form, City of Fort Collins Asbestos Statement Disclosure, City of Fort Collins Demolition Permit, City of Fort Collins Waste Management Plan and Documentation Form, and a Larimer County Miscellaneous Building Permit must be submitted. • Demolition debris, as identified by laboratory TCLP analysis, is non-hazardous and can be disposed of as standard construction waste at an approved landfill. • If any additional materials not sampled as part of this assessment are encountered or the scope of work expands/changes, additional sampling must be completed, or the material must be assumed asbestos containing. • LTE recommends that when demolition of the building is completed, the ground and soil surrounding the building be free of debris and material waste. The City should note that they are responsible for notifying the demolition contractor of materials that contain less than 1% asbestos that are to remain during the demolition process. It is the contractor’s responsibility to ensure worker protection from exposure to the asbestos in these materials that remain in the building during demolition. Ultimately, it is the owner’s responsibility to ensure that their contractor uses appropriate demolition methods to prevent releases of asbestos fibers. LIMITATIONS This report is exclusively for the use and benefit of the City of Fort Collins and is not for the use or benefit of any person or entity. No survey can completely eliminate the uncertainty regarding the presence of asbestos-containing materials and other hazardous materials. The level of diligence and investigative procedures used are intended to reduce, but not eliminate potential uncertainty regarding the presence of these materials. The procedures used for the survey attempted to establish a balance between the competing goals of inspection cost, time, and aesthetic damage. The determinations of this report should not be construed as a guarantee that all such materials present in the subject property have been identified in this report. The inspection was conducted in a manner consistent with the level of care and expertise exercised by members of the asbestos inspection and assessment profession. Those entities and persons involved with the inspection and generation of this report do not imply or guarantee that all potential ACM on or in the structure have been identified or sampled. Historically, asbestos has been added as a component to over 3,000 materials and products produced in the United States. Informational (hazard communication) training for demolition supervisory staff and workers and additional inspection by an accredited asbestos inspector during renovation may assist in identifying any other hidden or concealed materials that may exist in the building.
