HomeMy WebLinkAboutCORRESPONDENCE - PURCHASE ORDER - 9185845Official Purchasing Document Last updated 3/2018 Page 1 of 4 AMENDMENT #1 GRANT AGREEMENT BETWEEN THE CITY OF FORT COLLINS AND NORTHERN COLORADO HEALTH SECTOR PARTNERSHIP This First Amendment (Amendment #1) is entered into by and between the CITY OF FORT COLLINS (the “City”) and NORTHERN COLORADO HEALTH SECTOR PARTNERSHIP (the “Recipient”), with the Fort Collins Area Chamber of Commerce, acting as fiscal agent. WHEREAS, the Recipient and the City entered into an Agreement effective April 9, 2018 (the “Agreement”); and WHEREAS, Recipient and the City desire to amend the Agreement to incorporate additional Work and grant funds. NOW, THEREFORE, in consideration of the foregoing recitals and the mutual promises herein contained, the parties agree as follows: 1. Exhibit A, Scope of Work, is amended to include the additional services described in the attached Exhibit A-1, consisting of one (1) page. 2. Exhibit B, Project Budget is amended to include the additional funds as described in the attached Exhibit B-1, consisting of one (1) page. 3. Paragraphs 3 and 5, on page 3 of the Agreement, are hereby amended to revise the Project completion date and receipt submittal date to December 15, 2019. 4. Paragraph 4, on page 3 of the Agreement, is hereby amended to add a second midyear written evaluation deadline of the Project for work completed in 2019 that the Recipient must provide the City by no later than August 31, 2019. 5. Paragraph 14, on page 4 of the Agreement, is hereby amended to add the payment schedule for the additional services as follows: two payments of $7,500 each, the first payment will be made after execution of Amendment 1. The second payment of $7,500 will be made after submittal of the second midyear written evaluation. Invoice will be payable by the City Net 30 days from receipt of the invoice. Except as expressly amended by this Amendment #1, all other terms and conditions of the Agreement shall remain unchanged and in full force and effect. In the event of a conflict between the terms of the Agreement and this Amendment #1, this Amendment #1 shall prevail. Program Areas Peer networks: opportunities to interact with peers. Examples include CEO roundtables, occupation-specific professional development workshops, and industry-specific, topical symposia. Amount requested: $500 Projects proposed: PASRR Certification Event, Northern Colorado Health Sectors Partnership Monthly and Quarterly Meetings_ Talent: increased access to qualified workers. Examples include customized training programs for key occupations, marketing to build Fort Collins brand in industry, informative relocation-oriented website geared at specific talent segments, collaborative recruitment. Amount requested: $14,000 Projects proposed: Behavioral Health Scholarship Pilot, High School Internship Program, Northern Colorado Health Care Job Fair, Medical Program Scholarships Markets: increased visibility in market segments and improved market penetration. Examples include export/trade assistance, co-marketing opportunities at trade shows, trade delegation trips, and general marketing activities that promote a Fort Collins brand for the industry. Amount requested: $500 Projects proposed: _Maintenance of nocohealthsector.org and nocohealthsector.com websites. City of Fort Collins featured prominently on the homepage as a key sponsor of health care sector efforts in the region.
