Maximum Fee: (time and reimbursable direct costs): $72,577
Protect Description:
Relocate the Laboratory Building at 222 LaPorte Avenue to the east approximately 100 feet and to the north
5 feet as shown in the new plans for the Utility Administration Building.
Scone of Services:
The Construction Manager/General Contractor (CM/GC) will develop a plan for relocating the Laboratory
building at 222 Laporte Avenue. This plan will need to be approved by Landmark Preservation Commission
and then put Into place and carried out once the new footings and foundations have been completed for the
Laboratory building at the new location. The relocation plan will be submitted to the City of Fort Collins and a
permit issued prior to work beginning. The CM/GC will need to Incorporate any items necessary maintain
stormwater requirements during the scope of work. General condition charges will be allowed as per Exhibit
C of the RFP along with the approved rates. Each invoice shall have detailed itemization of each item
submitted and no contingency spent without Owner approval. Total Invoice to be within the not to exceed
amount as listed above.

escort if required by state, etc.
All work to be completed in a workmanlike manner according to standard practices. Any alteration or deviation
from above specifications involving extra costs will be executed only upon written orders and will become an extra
charge over and above the estimate. Building cannot be transported or work performed during inclement
weather, temperatures under 32 degrees or when roads or earth are unsafe and/or unstable for travel.
Mammoth Structural Movers, Inc and its employees will NOT be responsible for any personal effects left on
premises during these processes. Please make sure that anything of sentimental value or otherwise is removed from
the premises before work commences. The Owner is to carry fire, tornado, vandalism and other necessary insurance
not covered by our policies.
Mammoth Structural Movers, Inc. will not be responsible for inherent structural damage. Mammoth will make
every effort to preserve plumbing and wiring that is left in place but will NOT be responsible for any damage in our
efforts to move this structure. Mammoth will make every effort in order to preserve landscape and sidewalks, etc
but does not guarantee it from damage during the move processes. Please make sure any important landscape
features arc removed before Mammoth work commences. Access to these areas needs to be provided to Mammoth.
After three business days from date of signing this paperwork, deposit is nonrefundable and will be applied to
mobilization. In the event a subcontractor agreement is required for this work, Mammoth Structural Movers, Inc.
will require our terms to be included in your paperwork.
Acceptance of Proposal The above prices, specifications and conditions arc satisfactory and are hereby accepted.
You arc authorized to do the work specified. Payment will be made as outline above.

Relocate building to the South east corner of property and set on new foundation provide by others

Moving contractor will remove concrete slab and excavate inside of building for relocation; disposal of
slab debris and excavated dirt to be done by others
Exclusions -
Utility disconnect and reconnections
New excavation and new foundation
Any site work need for building relocation
Engineering
Permits
Bonds
Any plumbing, heating, and electrical work
Removal of existing foundation
Disposal of any debris created by moving operation
Any asbestos or lead paint removal, abatement, testing and permitting
Any changes to insurance other than currently carried by Ryberg Construction Co Inc. Current insurance
certificates available upon request
Customer is to supply adequate and acceptable delivery site for building relocation. All foundation work
must be coordinated with mover prior to foundation work commencement. Any foundation work
performed without coordination with mover or not done to mover requirements may result in project
delays and additional charges.
Note- Effective January 1, 2015, All, moving contract must be on movers form and sign by property
owner. No third party contract will be allowed.

PROPOSAL/AGREEMENT
Thank you for the opportunity to provide you with a bid to relocate your structure. After an on -site consultation we
have an option available for this relocation process.
Project Name and Address: Butterfly Building — 222 La Porte, Ft. Collins, CO
Destination Address: This is an on -site move.
1) Provide all necessary materials, labor and equipment to rig and prepare structure listed above for
relocation.
2) Demolition of the interior slab.
3) "Frame -In" where the windows are being removed on the building.
4) Relocate structure to the destination provided above.
5) Set structure at new site.
6) Means & Methods for the City.
Proposed price: S 45,000.00 for structure relocation.
Terms of Payment: 10% of proposed price upon signing this agreement.
40% of proposed price upon commencement.
30% of proposed price when building is lifted.
Balance of proposed price upon commencement of structure set.
"*Please initial that you understand and agree to the financial terms of this Agreement.
Owners Responsibilities:
1) It is the property/structure owners' responsibility to know what is required of them in each
municipality and to comply with all the city and county standards/codes required to relocate
structures and adhere to these requirements.
2) All building inspections, asbestos reports, asbestos abatements, demolition permits, building site permit, etc.
as necessary by local municipalities.
3) *"Secure your building" in order to prevent vandalism or theft, etc.
4) All utility disconnects in and around structure before Mammoth commences on job.
5) Old and new site debris removal.
6) Assist in the coordination and pay for costs of any necessary utility movement during relocation phase.
7) Provide any dirt work at the new site.
8) All debris removal.
9) Remove windows.
10) All access and egress and the costs associated with the movement of your structure. Including but not
limited to overhead wire movement, traffic light movement, signs, right of way access, fencing
