The reason for this contract change is to allow Project and Construction Management oversite to
complete the Council Tree Library remodel. Ditesco was originally contracted to provide project and
construction management services on the Library Streetscape and Library Diagonal Walk projects, as
well as on -call services for the Old Town Library Expansion Project close out. The construction remodel
of the Council Tree Library is scheduled to be completed by January 2015. The original scope of work
and agreement Is attached hereto as Attachment B,
2. Description of Change:
The description of the contract change is attached hereto as Attachment A
The contract change includes creation of bid documents, facilitation of the bidding process, resident
engineering services during construction, and close out procedures
3. Change In Contract Cost: $5,593.00
4. Change in Contract Time: Contract time for this work Is extended to March 1, 2015.

Project Management/Owner's Representative Services

We are pleased to present you with this summary of anticipated scope of services:
• Create final bidding package contract documents.
• Facilitate bidding process, including contacting potential bidders, conducting a pre -bid walk through,
collection of final bids, and bid award.
• Facilitate pre -construction meeting.
• Oversee, manage, and coordinate construction amongst Successful Bidder, City On -Call contractors, and
sole -source supplier contractors.
• Review and process Pay Applications through the Library District.
• Perform routine inspections of the work; ensure conformance to contract documents.
• Process project close out, including completing punch list walk through and follow up.
• Other miscellaneous services as requested by the Library District.

RE: Project and Construction Management Professional Services Agreement
Building Expansion Project
Dear Ken:
We are pleased to present you with this Agreement for Professional Services for the Library Expansion Project.
It is our understanding that Poudre River Libraries will be completing an 11 month walk through of the Old Town
Library Building Expansion project. As previously discussed, Ditesco staff will be utilized in an on -call hourly basis
at the discretion of the Library. Our team will be readily available throughout the completion of the project to provide
oversight, inspection, and owner representation to ensure your needs are met. Attached is our Professional
Services Agreement, as well as a Ditesco Wage Rate Schedule. Please review as necessary. If you find
everything in order, please execute and return 2 copies to my attention. I will return a fully executed copy for your
records.
