ADDENDUM NO. 1
SPECIFICATIONS AND CONTRACT DOCUMENTS
Description of BID 7649: DENTAL ADMINISTRATOR
OPENING DATE: 3:00 PM (Our Clock) June 13, 2014
To all prospective bidders under the specifications and contract documents described
above, the following changes/additions are hereby made and detailed in the following
sections of this addendum:

EXHIBIT 1 – QUESTIONS AND ANSWERS
1. Can you advise what the ASO fee for your current plan with Delta is?
The current ASO fee is $4.05 per employee per month.
2. How long have you been with Delta?
Fifteen (15) plus years.
3. Have there been any plan changes?
Plan provisions have remained consistent for the last three (3) years.
4. Can we get a detailed SPD to better match the plan design with limitation and
exclusions? Will you provide the full dental certificates?
SPD attachment will be sent to qualified firms via secure email.
5. Is more experience available?
Experience Summary will be sent to qualified firms via secure email.
6. We are requesting a "provider utilization report" from Delta Dental showing the
dentists that the group has seen over the last year. They should be able to provide this
with claims information included as well. Delta normally can provide this report also
showing which providers are in the premier network vs the PPO which would help us as
well.
This information is not part of the RFP review and would not be a deciding
factor for the City of Fort Collins. Therefore, we will not be following up with
Delta Dental for this information.
7. The Proposal requirements indicate performance guarantees, but I do not see them
in the material. Will you be providing them?
We won’t be providing current vendors’ performance guarantees. Proposals
should include recommended performance guarantees.
8. With regards to the geoaccess report: The criteria states 2 in 10 miles for dentists.
Do you want it broken out by dentist, specialist, Endodontists, Oral Surgeons,
Orthodontists, Pedodontists, Periodontists, Prosthodontists. Would you like them
grouped in any specific way?
It would be preferred to break them out by dentist, specialist, Oral Surgeons, etc.
9. Can you please advise if the City of Fort Collins is ERISA or Non-ERISA?
Non-ERISA.
10. Can you please advise what the current employer contributions are by tier for the
Dental PPO plan? It appears that this information was provided for 2013, but not for
2014.
No changes in 2014.

11. Can you please advise what the current out-of-network reimbursement levels are
for the Dental PPO plan? Is it R&C or is it MAC for both Premier and Out-of-Network
dentists?
Premier and Out of Network are reimbursed based on the Premier provider
maximum plan allowance. So it’s based on a MAC of the Premier providers –
not R&C.
12. Can you please provide the 2014 Premium Equivalents?
Employee only= $33.23, employee + spouse= $59.81, employee + child(ren)=
$74.77, family= $99.69.
13. Can you please provide a dental claims report that includes the following
information:
a. Provider Name (first & last), address, city, state, zip code and Tax ID #.
b. Amount Charged, amount paid, number of claims and number of services.
It is not standard practice for the City to disclose this information; therefore we
must decline to provide the specific claim information.
