SPECIFICATIONS AND CONTRACT DOCUMENTS

To all prospective bidders under the specifications and contract documents described
above, the following changes/additions are hereby made and detailed in the following
sections of this addendum:

QUESTIONS & ANSWERS
1. Please provide copies of current contracts.
Information Included in additional secure files.
2. With regards to the LTD do we know if the group participates in social security and or PERS
(public employee retirement)? Are there Retirees?
We participate in Social Security not PERA. No Retirees.
3. Can we get enrollment history for all lines?
Please see number of active lives on June 2014 bill provided by CIGNA (included in
additional secure files). Since the City is self-administered, CIGNA does not house any
enrollment history reports.
4. Is rate history available on the basic life, or has the group had a rate of .12 since 1/2010?

5. Does the LTD have EAP, Family Care, or COLA?
3 EAP Counseling sessions included.
6. What are the AD&D and VAD&D Benefits (i.e., Seat Belt/Air Bag, Education, Child Care,
etc.)?

7. Do you have the Voluntary Life/VAD&D Rates?
Included in additional secure files.
8. Can we get 2014 STD claims numbers? 2012 and 2013 have been provided.

9. Does Benefit = Gross Benefit or Net Benefit on the claims listing? Please provide whichever
has not been provided.
The LTD benefit is the gross disability benefit on the experience reports. Offsets cannot
be broken out.
10. RFP outlines that uniformed police and fire department employees are eligible to participate
in the plan after they are no longer eligible for FPPA benefits, which is after 25 years of service
and age 55. Census shows some employees under the age of 55 as participating in the plan, is
this accurate? Would those employees that no longer participate in the FPPA plan then be
eligible for this plan with a 60% benefit?
There are also Police and Fire Civilian employees included in the census who are not
eligible for FPPA and only eligible for LTD benefits at the City rate. There should be no
Sworn Police and Fire participating in the plan prior to age 55.

11. Would you please clarify who is eligible for ASO STD, and how I can identify these
employees on the census?
All employees included in census except those with a benefit group of CN, CNP, PF,
RPC, RT, and TP (439 not eligible).
12. Please provide LTD reserves on the LTD claims listing. Also, STD claims dollars would be
beneficial as they are requesting a fully insured quote.

13. Please provide voluntary life rates.

14. Please provide a detailed open & closed claim listing for LTD with Date of Birth, Date of
Disability (and date of closure for closed claims), gender, amount paid to date, and claim
reserve (for open claims).

15. Please provide the RFP in Microsoft Word format to facilitate answering the Questionnaires
in the document.
The City issues all RFP’s and corresponding documents via BuySpeed, which only
accepts pdf file attachments.
16. Will we be assumed to agree to the terms and conditions noted in Exhibit D if we do not
specify any anticipated deviations from the terms and conditions at this time?
Yes; however, the City may incorporate and/or modify items during contracting.
17. Can booklets be provided?

18. According the census, there are 1,153 employees in the ER paid LTD plan and 665 not on
the plan. Why are the 665 not in the plan? Or is that an error on census?
No error in the census. Some are not eligible because they are police, fire, or seasonal
employees. Please do not include the 665 in your underwriting.
19. Is the benefit amount on the claim LTD claim list net or gross?
The LTD benefit is the gross disability benefit on the experience reports. Offsets cannot
be broken out.
20. Is there portability? If so, does Basic Life have it?

21. Is there a Waiver? What is the plan design? Will it be for Basic and EE OGL?

22. RFP states G.I. is 100k. Is that for EE OGL and Sp OGL?
EE OGL. Included in additional secure files.
23. Please provide the EE OGL, Sp OGL and Child rate.

24. Please provide certificates.

26. Can we get the individual reserves for the Open claims on LTD?
Information unknown.
27. Can we get Gross benefit? There is a report with a column called Benefit, but we would like
to confirm if that is Gross or Net Benefit.

28. Can we get the class that each open claim is in?
Information unknown.
29. For the Leave Management, is there any experience data that can be provided? We would
like to get Leave incidence for the past 12 months?
Information unknown.
30. What is the number of Payroll Lives for City of Fort Collins to which Leave Management
Services would apply?
Information unknown.
31. Does the City participate in SS or the CO state plan?
See question #2.
32. For the LTD claim listing, is the benefit listed gross or net?

33. Can we get a more detailed open claim listing including offsets and total paid?
Information unknown.
34. Can you provide a date for the vendor selection announcement in September?
Exact date not known but the first week of September (2nd – 5th) – one of those dates.
35. The census has more than 200 employees that have no benefit amount and do not appear
to be included on the current billing statement. Twenty are City Retirees, who are not eligible,
and a few others may be new hires within the waiting period. However, those remaining appear
to be eligible based on salary, date of hire and occupation. Why do these employees have no
coverage?
Some are not eligible because they are police, fire, or seasonal employees. Please
do not include the 665 in your underwriting.
36. Please help us understand the census data. We believe:
Column R = Basic Life
Columns S-V = Employee Supplemental Life
Columns X-AB = Spouse Supplemental Life and matching AD&D
Columns AC-AD = Child Supplemental Life and matching AD&D
AE-AH = Employee Supplemental AD&D

37. Please advise what columns AI – AL on the census data represent.

38. Also on the census data, columns AM & AN appear to be Employee Only and Employee
plus Family VAD&D in increments of $10,000. Does the City of Fort Collins offer both
Supplemental AD&D (in salary multiples as listed in columns X-AH) and Voluntary ADD on an
Employee and Employee plus Family basis?
Disregard any mention to Voluntary Life (columns AM and AN).
39. Please provide a copy of the current AD&D policy(s) or certificate(s) so that our proposal
can match current plan provisions as closely as possible.

40. Do any of the AD&D coverages include any additional benefits?

41. Will the life insurance plan be self-administered/self-billed by the employer, or will the
insurance carrier be responsible for maintaining individual employee records and for generating
monthly invoices?
