Please let this letter serve to amend the following to the Services Agreement dated May 13, 2014 (the
“Agreement”).
The following changes/additions are hereby made and incorporated in the Services Agreement:
Exhibit B – Compensation, attached and incorporated herein by this reference, replaces the
original Exhibit B to incorporate updated revenue share percentages.
All other terms and conditions of the Agreement remain in full force and effect. The City looks
forward to continuing to work with Columbine Health.
