Amendment #02 to Professional Services Agreement
between The City of Fort Collins and Mogo Marketing+Media, Inc.
This Amendment (Amendment) is entered into by and between Mogo Marketing+Media, Inc. and
(the "Professional") and the City of Fort Collins, Colorado (the "City").
WHEREAS, Professional and the City have mutually entered into a Professional Services
Agreement dated June 24, 2014 (the "Agreement"); and
WHEREAS, the City desire to add a new Scope of Services; and
WHEREAS, Professional agrees to perform additional work;
NOW, THEREFORE, in consideration of the foregoing recitals and the mutual promises
herein contained, the parties agree as follows:
1. The Work Schedule. A new Exhibit "A", Scope of Services is added per the attached document
consisting of eight (8) pages.
2. Compensation. The City agrees to pay Professional a fixed fee not to exceed Five Thousand
($5,000). Payment shall be made by the City by December 17, 2014. The Professional shall
submit an invoice for payment.
Except as expressly amended by this Amendment, all other terms and conditions of the Agreement
shall remain in full force and effect.
IN WITNESS WHEREOF, the parties have executed this Agreement the day and year shown.
