RE: Miscellaneous Services Agreement - Utilites Payment Assistance Program
Dear Mr. Mendt:
The City of Fort Collins and Catholic Charities Northern entered into a Services Agreement
January 1, 2010. The Agreement Contract Period was one (1) year with the option for the City to
extend for additional one (1) year periods not to exceed four (4) additional periods.
Whereas the parties wish to extend the Contract Period one (1) additional year beyond the
original five (5) year period.
Now, therefore, the parties agree to extend the Agreement for one (1) additional year for the
period, January 1, 2015 through December 31, 2015.
If the renewal is acceptable to your firm, please sign this letter in the space provided, include a
current copy of insurance certificate naming the City as an additional insured for General
and Automotive Liability and return all documents to the City of Fort Collins, Purchasing
Division, P.O. Box 580, Fort Collins, CO 80522, within the next fifteen (15) days.
If this extension is not agreeable with your firm, we ask that you send us a written notice stating
that you do not wish to renew the contract and state the reason for non-renewal.
