We are very excited about this opportunity and look forward to working with the team. PROJECT DESCRIPTION Farnsworth Group proposes to provide LEED® Consulting Services, including LEED®certification coordination. The project will consist of an approximately 24,659 square feet of fitness activity area. . Farnsworth Group understands you would like assistance certifying the Foothills Activity Center (FAC) building at Foothills Mall under the U.S. Green Building Council's LEED02009 Guideline for Commercial Interiors. We have assumed about 12 months for the pro duration. For the purposes'of this proposal, we have assumed the goal is to achieve LEED Gold Certification. If protracted delays of longer than ninety (90) days occur beyond Farnsworth's control, we reserve the right to negotiate an equitable adjustment in compensation required for start-up, accounting and management expenses. Proposal is valid for 30 days upon date of issue. DISCLAIMER Farnsworth Group cannot guarantee the LEED® certification of this facility or any other for which we provide LEED®navigation or energy modeling services. LEED®certification and the number of points awarded for energy efficiency are solely the responsibility of the U.S. Green Building Council. Furthermore, Farnsworth Group does not recommend that the results from LEED®Compliance energy models be used for budgeting of energy costs. The technique outlined in ASHRAE 90.1 as well as the specific requirements for the models to meet LEED®criteria have limitations. Though the energy modeler makes every effort to model the energy usages as closely as possible, even the best assumption will lead to predictions that differ from actual usage. Proposal for LEED® Consulting Services RESPONSIBILITIES OF OTHERS The following services are required to be provided by you in order to allow Farnsworth Group to complete the scope of services outlined in Attachment B: Scope of LEED®Consulting Services: LEED®CONSULTING SERVICES • CM/General Contractor Responsibilities: 1. Provide the LEED®documentation and submittal information required to demonstrate compliance with all credits designated as their responsibility. 2. Complete LEED®Online letter templates as assigned. 3. Designate a single individual to be responsible for coordinating quality control and assurance activities with Owner and LEED® Consultant. 4. Provide the documentation, coordination drawings, and submittal information required. • A/E Responsibilities 1. Provide all drawings and specifications for review in Adobe PDF or equivalent format. 2. Provide timely responses to design review comments. 3. Incorporate sustainable design specification into contract documents. 4. Provide the submittal information required. 5. Provide the LEEDOdocumentation'and submittal information required to demonstrate compliance with all credits designated as their responsibility. 6. Complete LEED®Online letter templates as assigned. 7. Provide all'BoD documentation for review and comment. • Owner Responsibilities 1. Actively participate in the LEED®Consulting processes. 2. Provide Owner's Project Requirements and any available additional information regarding design intent. 3. Provide the LEED®documentation and submittal information required to demonstrate compliance with all credits designated as their responsibility. 4. Complete LEED®Online letter templates as assigned. FAC Building — LEED® Consulting 3 Farnsworth Group, Inc. Proposal for LEED® Consulting Services LEED® COMPLIANCE ENERGY MODELING SERVICES (OPTIONAL) • A/E / Owner's Representative Responsibilities 1. Provide energy modeler with required site, architectural, and systems drawings in AutoCAD or equivalent convertible format. 2. Provide energy modeler with required information not available from design documents in a timely manner. Provide documents in Adobe PDF format and drawings in AutoCAD or equivalent convertible format. 3. Inform energy modeler of any major changes to design in a timely manner. Provide changes in either Adobe PDF or AutoCAD format. FAC Building - LEED® Consulting 4 Farnsworth Group, Inc. Proposal for LEED® Consulting Services Farnsworth GROUP ATTACHMENT B: SCOPE OF LEED® CONSULTING SERVICES For this proposal Farnsworth Group's fees are based on the following items: LEED® CONSULTING SERVICES • LEED® Process Management 1. Design Phase a. Register project with USGBC LEED® 2009 ID+C certification and pay application fees at time of completion of design and construction applications. b. Review design documents and Basis of Design to assure LEED® prerequisites, credits and sustainable features targeted are incorporated. Provide recommendations to Owner/Architect/ Engineer to improve the sustainability of the facility. c. Review/verify specifications sections to ensure sound environmental quality, including sections LEED® Requirements, Construction Waste Management, Construction IAQ Management. d. Perform gap analysis of drawings and specifications sections required to achieve prerequisites and credits identified in LEED® 2009 ID+C Action Matrix. e. Review 90-95% construction documents to verify comments from previous reviews have been incorporated and the project requirements are clearly communicated. Attend design review meeting. f. Participate in the design review meetings during the design phase. For the purpose of this proposal, we have assumed four (4) meetings in Fort Collins, Colorado. g. Track LEED® scorecards and provide periodic updates to team. Update scorecards following each design review. h. Manage and maintain LEED® Online website for project, assigning tasks and responsibilities to correct team members and providing guidance on the supporting documentation required. Provide workshop explaining how LEED® Online templates work. i. Provide documentation and completed letter template for LEED® Accredited Professional. j. Compile required documentation for each design phase prerequisite and credit developed by Owner, Architect/Engineer and contractor for design phase credits. Provide QAQC review and comments prior to submission to USGBC for application. k. Submit Design Application to USGBC for review and approval of submitting credits. FAC Building - LEED® Consulting 1 Farnsworth Group, Inc. Scope for LEED® Consulting Services I. Distribute USGBC feedback and coordinate additional information requested through USGBC review (i.e. credits under question or audited). Coordinate with team to provide requested information. m. Serve as the Point -of -Contact for USGBC. Coordinate and manage all correspondence with USGBC regarding application and supporting documentation. 2. Construction Phase a. Attend pre -construction meeting and present LEED® responsibilities of the construction team. b. Verify LEED® compliance throughout construction phase, including Construction Indoor Air Quality and Waste Management. Review project submittals concurrently with design team to verify correct/necessary LEED® documentation has been included. c. Review LEED®progress reports as submitted by Contractor concurrent with pay applications. d. Compile required documentation for each construction phase prerequisite and credit developed by Owner, Architect/Engineer and Contractor for construction phase credits. Provide QAQC review and comments prior to submission to USGBC for application. e. Submit Construction Application to USGBC for review and approval of submitting credits. f. Distribute USGBC feedback and coordinate additional information requested through USGBC review (i.e. credits under question or audited). Coordinate with team to provide requested information. g. Serve as,the Point -of -Contact for USGBC. Coordinate and manage all correspondence with USGBC regarding application and supporting documentation. h. Assist Owner in processing acceptance form for the building upon LEED® certification award. i. To minimize greenhouse gases associated with travel, a total of 6 on -site meetings have been included for the construction phase. Additional meetings will be conducted via teleconferencing. FAC Building — LEED® Consulting 2 Farnsworth Group, Inc. Scope for LEED® Consulting Services Farnsworth 1 GROUP ATTACHMENT C: OPTIONAL - SCOPE OF LEED® COMPLIANCE ENERGY MODELING SERVICES LEED®COMPLIANCE ENERGY MODELING SERVICES We have outlined an optional scope of services for Energy Modeling as follows below. The scope of services and prices are based on the known information of the project. Energy modeling prices for LEED® Cl EA Prerequisite 2 and EA Credit 1.3 include the creation of both the proposed design model and the LEED®baseline model. The LEED®baseline model is developed per ASHRAE 90.1-2007. The prices also include two reports containing the results and predicted EAc1.3 points. Energy modeling does not begin until all necessary energy modeling parameters have been assembled. • Scope of Modeling Services 1. Modeler shall assemble all necessary information required for model from Owner, Design Team and issued design drawings. Questionnaires may be given to the design team to fill out to facilitate the modeler in gathering information. 2. Modeler shall implement most accurate weather profiles available; downloading existing weather files or creating new ones from historical data. 3. Modeler shall create building shell and orient to reflect location and existing terrain based on available or provided information. 4. Modeler shall implement realistic 24 hour, yearly usage profiles for all equipment, plug loads, people, controls and other parameters based on available or provided information. 5. Modeler shall implement lighting design based on available or provided information. 6. Modeler shall create as -designed mechanical and plumbing systems using realistic operating parameters. 7. Modeler shall create and implement equipment, plug loads, people, lighting, and mechanical and plumbing systems, based on the Energy Modeling Guidelines demonstrated in the LEED® 2009 ID & C Reference Guide. 8. Modeler shall implement actual rate structures from available utilities data. 9. Modeler shall calibrate the model based on benchmark data for comparable buildings. 10. Modeler shall implement energy usage/savings from on -site renewable energy sources. FAC Building - LEED® Consulting 3 Farnsworth Group, Inc. Optional - Scope of LEED® Compliance Energy Modeling Services • Baseline Building Energy Model 1. Modeler shall develop LEED® Compliant baseline energy model based upon Appendix G of ASHRAE Standard 90.1-2007 including changes to envelope, lighting and mechanical systems. 2. Modeler shall perform four separate simulations of the baseline model for each 900 rotation of the orientation based on LEED® requirements. 3. Modeler shall calculate baseline energy consumption and cost based on an average of the four simulations. • LEED® Submission 1. Modeler shall perform final energy model simulations based on final modifications to the models or slight changes to the design. 2. Modeler shall document energy model results in LEED® Online Template and provide all necessary supporting documents for submission to the USGBC. 3. Modeler shall address any questions subsequent from the USGBC review and resubmit energy model results as needed. FAC Building - LEED® Consulting 4 Farnsworth Group, Inc.
