Budget: $21,580
The City of Fort Collins Operation Services Dept. is requesting a sole source for Farnsworth Group to provide
Enhanced Commissioning services for the Foothills Activity Center at the Mall. Farnsworth is providing
Fundamental Commissioning services per the City's Building Department commissioning requirements for the
FAC (Foothills Activity Center) for the Foothills Mall developer Alberta. It is cost effective and in the City's best
interest to contract for these services with the same firm providing Fundamental Commissioning services. It
would be challenging and impractical to coordinate and harmonize work between the City's on -call
commissioning agent E-Cube for the Enhanced Commissioning and Farnsworth for Fundamental
Commissioning. By using one commissioning agent the City will realize pricing efficiencies and seem -less
progression between the stages of commissioning.
Upon your approval, the City will issue a purchase order for $21,580 to Farnsworth Group.

VIII. FEES FOR PROFESSIONAL SERVICES:
Based on previous project experience and anticipated work effort, Farnsworth Group
proposes to complete the work as outlined in the attached scope of services for the
lump sum fee, not to exceed as indicated below.

Reimbursable expenses including items such as printing costs, reproduction, travel
for trips to project site, etc. are included In the above fee.

Dear Mr. Co2zens,
Thank you for the opportunity to present you with this proposal for Enhanced Commissioning Services
for the Family Activity Center (FAC) building at Foothills Mall. As we may have mentioned, Farnsworth
has extensive knowledge and experience with the building Commissioning and Enhanced Commissioning
process.
As discussed, Farnsworth Group is proposing fees for Enhanced Commissioning Services as specked
under the LEED 2009 Guideline for Commercial Interiors. Attachment A outlines the scope of work.
We have based our fees on the following information/assumptions:
I. Project Foothills Mall Redevelopment, FAC Building— Enhanced Commissioning Services
2. Location: Fort Collins, Colorado
3. Facility Description: New Construction, Activity/Fitness Center, Approx. 24,659 total SF
4. For the purpose of developing this proposal, our scope and fees are based upon drawings
entitled "Foothills Mall Redevelopment, DCP NO.13 — 70% Construction Documents" dated
May 16, 2014 provided by the City of Fort Collins (for MEPs).
Below are the fees we are proposing for our services:
Commissioning Services: $ 21,080
Estimated Expenses: $ 500
Total Est. Fee: $ 21,580

Certified Commissioning Authority (CxA) by AABC and Building Commissioning Association
III. SCOPE OF COMMISSIONING (Cx)SERVICES
A. LEED Prerequisite EAp1 Fundamental Commissioning of the Building Energy Systems,
in conjunction with Commissioning specified by the City of Fort Collins Amendments
to the 2012 International Building Code, shall be executed under a different contract.
B. LEED EAc3 Enhanced Commissioning Services
1. CxA shall conduct at least one design review of the Owner's Project Requirements
(OPR), Basis of Design (BOD), and design documentation and back -check review
comments in subsequent design submissions.
2. CxA shall review contractor submittals applicable to the commissioned systems for
compliance with the OPR and BOD documents. The review shall be concurrent
with the review of the architect or engineer of record and submitted to the design
team and Owner.

3. CxA shall develop a Systems Manual that provides operating staff with the
documentation and information needed to operate the commissioned systems
and equipment.
4. CxA shall provide a Commissioning Report to include an executive summary of the
commissioning process, identification of issues and resolution process, system
performance test results and evaluation, and confirmation of systems meeting the
criteria outlined in the OPR and BOD documents. In addition, the Commissioning
Report shall provide summaries of the design review, contractor submittal review,
and 0&M documentation and training review processes.
5. CxA shall review and verify that training of operating personnel on the
commissioned systems has been completed.
6. CxA shall conduct a review of the operation of commissioned systems with
operations and maintenance staff within 10 months after substantial completion,
and shall include a plan for resolving remaining outstanding commissioning -related
issues.
IV. SYSTEMS TO BE COMMISSIONED
A. One Roof Top Unit (RTU-1) with heat recovery wheel and associated controls
B. One rooftop Dedicated Outside Air System (DOHS-1) with associated controls
C. One rooftop Variable Refrigerant Flow (VRF) Condensing Unit (CU-1 A-C) with
associated controls
D. Twenty-five Variable Refrigerant Flow (VRF) Fan Coil Units (FCU-1 thru FCU-25)
E. Fifteen Variable Refrigerant Flow (VRF) Selector Boxes (BS-1 thru BS-15)
F. Four Destratification Propeller Fans (DF-1 thru DF-4) with associated controls
G. Two Cabinet Unit Heaters (CUH-1 and CUH-2) with associated controls
H. Review of supply, return registers, grilles and their locations
I. One Water Heater (WH) and circulation pump
J. Lighting System Controls
V. RESPONSIBILITIES OF OTHERS
A. GC Responsibilities:
1, Construction Manager General Contractor (GC) shall attend the Commissioning
Kick -Off Meeting.
2. GC and each subcontractor shall designate a single individual to be responsible for
coordinating quality activities with Owner.

3. Inclusion of the Commissioning requirements in the Specifications and Contract
Documents shall be the responsibility of the design team.
4. GC shall forward a copy of approved submittals to the CxA with additional
manufacturer's installation instructions, operation and maintenance manuals and
Warranties.
S. GC shall complete Engineer of Record's MEP punchlist items before the start of
Functional Performance Tests (FPT).
6. Distribute to CxA RFis and ASIs that impact specific systems to be commissioned.
7. GC shall coordinate adequate personnel and tools for FPT tests and retests (if
required).
B. Provide Test and Balance (TAB) contractor's reports for CxA review before the start
of FPT tests.
9. Coordinate schedule with TAB contractor prior to verification by statistical
sampling in the presence of CxA.
10. Building pollutant flush -out as required by the 2012 IBC as amended by the City of
Fort Collins.
B. TAB Contractor Responsibilities:
1. TAB Contractor shall verify the following (in general terms):
a. Accessibility of equipment and components required for TAB Work.
b. Adequate number and placement of duct balancing dampers to allow proper
balancing while minimizing sound levels in occupied spaces.
c. Adequate number and placement of test ports and test instrumentation to
allow reading and compilation of system and equipment performance data
needed to conduct both TAB and commissioning testing.
d. Air flow rates have been specified and compared to central equipment output
capacities.
2. Contractor shall assign adequate personnel for FPT tests, and as required for
scheduled retests.
C. HVAC Contractor Responsibilities:
1. Verify HVAC systems including instrumentation and control systems, subsystems,
and equipment have been completed, calibrated, and started.
2. During the installation checklist, the subcontractor shall collect nameplate data,
unit capacities, HP, model numbers, and serial numbers for all equipment
electronically for input into equipment installation checklists.

3. Contractor shall assist CxA too btain trends of the system operating parameters to
evaluate acceptable system functionality. The requirements of trending shall be
specified with FPT procedures. Contractor shall establish these trends, ensure they
are being stored properly, provide CxA web -based remote access, and forward the
data in electronic format to the CxA, before the start of FPT tests.
4. Contractor shall assign adequate personnel for FPT tests, and as required for
scheduled retests.
S. Facilitate functional performance testing of systems and verify intersystem
performance after approval of test checklists for systems, subsystems, and
equipment. Procedures include setting systems, subsystems, and equipment into
operating mode to be tested.
6. Prior to commissioning any portions of the controls system, the subcontractor
shall verify that the portion to be commissioned has been installed per plans,
specifications, and approved shop drawings. All cabinet doors, J-boxes, actuators,
sensors, controller outputs, transducers, etc., shall be labeled per specifications
prior to commissioning.

E. Electrical Contractor (EC) shall provide personnel to demonstrate lighting controls.
VI. ADDITIONAL SERVICES/EXCLUSIONS:
A. ADDITIONAL Cx SERVICES AVAILABLE, NOT INCLUDED IN THE SCOPE OF WORK
1. Field on -site verification of TAB measurements for facility utilizing either TAB
Contractor or Farnsworth Group measurement equipment.
2. CxA services to -develop the BOD and/or OPR documents will be provided for an
additional fee.
B. SERVICES EXCLUDED FROM THE SCOPE OF WORK
1. Cx services required for additional testing of individual systems beyond initial
testing due to failure of contractor to correct deficiency after initial test.
Additional CxA services will be charged as a change order.
2. Additional site visits required by rescheduled start-up testing or functional
performance testing due to project delay or missed scheduled construction
milestones.
3. Additional testing required if it is determined that the system is constructed
according to the Contract Documents, and Owner decides design modifications are
required to bring the performance of the system to the OPR and BOD documents.
4. Indoor Air Quality verification or testing.

Farnsworth Group cannot guarantee the LEED certification of this facility or any other
for which we provide services. LEED certification and the number of points awarded
for commissioning and energy efficiency are solely the responsibility of the U.S. Green
Building Council.
