Description of RFP: RFP# 7601 Health & Wellness Program Provider-Senior Center
OPENING DATE: 3:00 PM (Our Clock) March 14, 2014
To all prospective providers under the specifications and contract documents described
above, the following changes/additions are hereby made and detailed in the following
sections of this addendum:

EXHIBIT 1 – QUESTIONS & ANSWERS
1. Will there be a charge for rent?
No.
2. Based upon a bullet item under GOALS on page 3 of Request for Proposal (RFP)
document– are you expecting a physician or a mid-level (NP/PA) onsite while open?
Only time it would be expected would be if it is a requirement to have a physician or
mid-level (NP/PA) on site for the activity being provided.
3. Do you want health & wellness programming for children?
The City is receptive to consider proposals with programming for children, however,
this is not a focus area for this RFP.
4. Based upon the hours of operation, are you expecting programming in the evenings
and on the weekends?
Yes, patrons may request massages, foot care and other offered services during
evenings and weekends. The City is seeking proposals which support services
consistent with the Senior Center hours of operation.
5. Will we be responsible for the Volunteer of America (VOA) Senior Nutrition Program?
If you want to put a proposal to oversee this program please indicate that. If you
are fine with the City still overseeing the program please indicate that in the
proposal.
6. Can our RFP team have a tour of the facility expansion?
A tour has not been scheduled. The City will advise if one is scheduled.
7. What is City’s revenue share expectation? Percentage?
This is an element of the selection criteria and up to each firm to
determine.. However, in Project Manager’s past experience it usually ranged from
60 – 70% for provider of service and 40 – 30% for the City. Include your proposed
revenue share percentages in your proposal.
8. May we include fitness classes in our offerings? We may be able to move a current
program to the Senior Center – this is currently supported by 200 older adults.
The City would be willing to take a look at this as proposed program. However, the
firm selected will not have exclusive rights to conduct fitness classes. The City
would continue to offer other fitness class programs such as Silver Sneakers,
Zumba, Yoga and etc.
9. Does the space offer cooking facilities?
Facility has a kitchen in which cooking classes are held. It is not ideal for a true
cooking class. However, again the City could consider offering cooking
classes. The City could also look at portable cooking stations in the future.

10. On page 6 of the RFP document – under the heading “On-site Health & Wellness
Program Provider General Information” – question #2 – “How many other similar
programs are currently being serviced by your firm, from where?” – We are not certain
what exactly you are asking?
Basically, what is your experience in running health and wellness type
programs? And do you have any similar agreements or partnerships in place?
11. Can you explain your fee structure? In reviewing the RECREATOR we see
membership, drop in, punch card, discounts for classes. Can you explain these levels
and how/why?
Here is a quick overview of the Senior Center current pricing and discounts.
Annual Senior Center Membership
60+ $25
59 and under $50
Senior Center Members receive discounts on programs that are identified in the
RECREATOR. These are the members we are referring to in the RFP that would or
could receive discounts on the health and wellness programs.
Senior Center Members does not grant patrons access to the swimming pool,
cardio/weight room, open gym, and walking track.
Senior Center Member benefits:
-- $3 per month discount on senior pass or multi-facility pass or $1.00 off daily
admission to use fitness facilities
-- Unlimited use of:
-- Free admission to drop-in programs such as: billiards tables, art studio,
library/media center, bingo, cards and bridge
-- Member celebrations (for 50+ members only)
-- Discounts on select services and classes, including trips and travel, outdoor
recreation, special events and more
-- Two day advance registration for classes
-- One guest pass per month for daily admission (upon request)
-- Opportunity to rent community garden plots
-- Free limited legal counseling and Notary service (for 50+ members only)
-- Brief health chat with health district nurse and blood pressure check (for 50+
members only)
-- Opportunity to join Member-Only clubs (for 50+ members only)
-- C.H.A.T. and Quilting Quorum (free and for 50+ members only)

Pass holder benefits:
Open Gym, Weight & Cardio Area, Walking/Jogging Track, Open & Lap Swim,
Locker Rooms, Billiards Room, Library, 50% off of Land and Aqua Classes held at
any Recreation facility.
Daily Admission
This admission fee allows the same facility access as the Senior Center Unlimited
Pass for one day. This is for citizens or visitors that want to pay a daily use fee.
