Apparent successful bidder to receive:
• Form 605, Form 621, Form 715, and Form 718
• These forms must be returned by 4:30 pm the following day
(currently Jan 7, 2014)
Monthly payments will not be processed until all documentation
required by CDOT for federally funded projects has been
submitted to the City.

PROJECT OVERVIEW
PHASING
• MILESTONE DATE for Phase 1 – Section 520
• All Phase 1 work must be completed and accepted by ENGINEER on or before
April 30, 2014.
• Subject to Liquidated Damages of $1,400 per calendar day
• Phase 2 Work – Section 520
• Phase 2 and all remaining work will ONLY commence upon delivery of the
existing 165’ bridge to the project site, unless approved otherwise in writing
by the ENGINEER.

PROJECT OVERVIEW
• Coordination with utilities and others
– PRPA pole relocation work – T.B.D.
– CDOT bridge replacement project
– Contractor will need to coordinate with any utilities that may be
in the work area.
• Contractor responsible to acquire all appropriate permits and submit
required documentation
• City Flood Plain Use Permit – Provided by Owner, transferred to
Contractor
• Erosion Control
• CDOT Form 205 for each subcontractor

• PROJECT MEETINGS:
• Once a week. Location and time to be determined.
• Agree to weekly quantities
• PROGRESS AND SCHEDULE REVISIONS:
• Prior to construction, initial schedule must be submitted and
approved
• Schedules shall be updated weekly
• The Contractor may be required to submit additional schedules
depending on the progress of the project.

The completed project work is subject to FEMA review and acceptance.
All work must be completed to tolerances identified in section 625.
Tolerances must be validated by a Colorado Licensed Professional Land
Surveyor to the ENGINEER’s satisfaction prior to acceptance.

Contractor will be required to keep and submit record drawings
Contractor must provide a stamped as-built survey, from a CO.
licensed surveyor for approval.

however, the contractor is responsible for installation of erosion

followed.

PHASE II – Setting Bridges
• Take delivery of existing re-purposed 165 foot bridge span
(delivered in two sections) from CDOT Mulberry bridge project
(Approx. late October). City and contractor to inspect and then
contractor takes responsibility.
• Set new fabricated 100 foot bridge span.
• All connection hardware will be inspected. Contractor will need
to supply new elastomeric bearing pads for repurposed bridge.
