To all prospective bidders under the specifications and contract documents described
above, the following changes/additions are hereby made and detailed in the following
sections of this addendum:

1. What are the number of new claims filed in:

2. How many of these claims were protestable separations and how many were not
protestable separations for:

Response: 2011 - 6

4. What is the City's current method for financing their unemployment program?
Reimbursing, flat tax rate or calculated tax rate?
Response: Reimbursing
5. Does The City of Fort Collins currently outsource this service or handle in-house?
Response: It is currently outsourced.
6. Will a file feed be available? If so, what information will be provided on the feed?
Also, what is the frequency of that feed?
Response: No a file feed is not available.
7. What payroll and HRIS system and provider are you using today?
Response: JD Edwards/Oracle.
8. Please provide the total number of claims processed in 2012 :
Response: 99.
9. Of those, how many were voluntary quits? Discharges?
Response: Voluntary quits: 21 Discharges: 12
10. Total number of claims not protested (Voluntary reduction offerings,
displacements, contract losses)?

11. Of the Non Protestable claims are they not protested because of the separation
reason or because the employer has chosen not to protest?
Response: Both
12. Can you provide a case analysis (category summary breakdown) of your claims
not protested (poor performance, lack of work, not protested at client request, no
sep info provided, not protested by state regulation, other claims not protested,
etc.) for a recent annual period?
Response: Lack of work, (50) or not protested at client request (4)
13. Total number of hearings scheduled? Of those how many were “No Appearance”
hearings and “Petition for Appeal” hearings?
Response: Hearing scheduled: 6 City always appears for hearings. In 2012
all claimants appeared.
14. Total number of hearings attended? Of those, how many were attended with 3rd
party representation?
Response: 6 – all had 3rd party representation
15. Is it your preference to have your unemployment provider represent you at all
unemployment hearings or do you prefer to manage your own hearings with the
option to request representation for an additional fee per hearing?
Response: Preference is to have unemployment provider represent the City
at all unemployment hearings.
16. If your current provider is providing only a la carte hearings representation for your
today, what type of information and support do they provide in passing the
hearings back over to you for hearings that you will be handling on your own?
Response: Current provider supports City in all hearings.
17. Will there be a central point of contact when requesting separation details?
Response: Yes
18. Where is the supporting documentation and separation information currently held?
Response: Centrally located in Human Resources
19. How many HR Contacts will be involved in investigating the protestable claims.
Response: 1-2
20. Will the vehicle for communication with the employer be email?
Response: Yes

Response: In-house
22. Please provide your annual projected breakdown of employment/income
verifications. Number of private verifications? Number of government verifications?
Response: N/A all handled in-house and will continue to be
